Position Summary The Care Coordinator is one of the integral roles of the Integrated Recovery Program (IRP) team. The care coordinator works with other members of an interdisciplinary team to ensure participants have the support they need in order to be successful in their recovery from substance use or abuse. The position provides this support in the form of facilitating linkages to other programs, providing referrals for services internal and external to Mary's Center, as well as working with pharmacies and insurance companies to ensure participants have easy access to medication assisted treatment. The position requires an openness to and understanding of co-occurring disorders treatment, trauma-informed care, and a harm reduction approach. Reportability This position will report directly to the Addiction Specialist and Care Coordinator Supervisor. Language Requirement - Ability to communicate effectively in English is required. Additional language proficiency or fluency preferred. Physical Demands - Regularly required to sit; frequently required to reach with hands and arms, walk, stoop, kneel, crouch, talk or hear; must be able to lift objects up to twenty-five (25) pounds. Work Environment - Mostly in a typical office and/or telework setting with quiet to moderate noise level. May be exposed to extreme cold, heat, and humidity due to outside weather conditions.
