Job Number: 24-5494 / 24-6863 / 24-5425 / 24-5937 Job Summary: Integral Care has three ACT teams that support clients with complex mental and physical health needs, who frequently use emergency health services and/or interact with the criminal justice system. Integral Care's ACT team includes social workers and counselors who focus on mental health and case management, a housing specialist, a substance use counselor, a peer support specialist, a registered nurse and an advanced practice nurse. The team serves adults who live with mental illness and substance use disorder. Focusing their care on three specific areas: frequent and intensive psychiatric emergencies, chronic homelessness and frequent involvement with the criminal justice system. The ACT teams fit their services to the needs of the client. The complexity of those needs require the team to be involved in all aspects of client care. Including, routine healthcare visits, crisis episodes, incarcerations and psychosocial needs including housing, employment and family support. The team promotes health and well-being by providing access to treatment, rehabilitation and support services. Services include 24-hour mental health crisis support, one-on-one emotional support, medications, housing support, access to medication, nursing services, drug and alcohol treatment and help building daily living skills. The team brings services to clients at home or in the community. Regular, predictable attendance is considered to be an essential requirement for this position. Other duties may be assigned. Hours: Full-Time. Workdays: Monday-Friday (Rotating on call after hours/weekends/holidays). Workhours: 8:00AM-5:00PM.
