Primary Purpose

Responsible for ensuring that patients are scheduled for follow-up at the correct locations/clinic. Ensures that patients are contacted in advance to be reminded of scheduled appointments.

Shift:The position requires they work 5 days a week. 8-hour shifts.7am-3:30pm. Rotating weekends and holidays.

Education

*High School Diploma or GED.

Experience

*Must have two years related experience.

Skills or Special Abilities

*Must be able to communicate effectively with adolescents, families and clinic staff orally and in writing.

*Must have good problem analysis and interpersonal skills.

*Must have working knowledge of community resources of benefit to client.

*Must demonstrate patient centered/patient valued behaviors.

*Spanish/English bilingual skills strongly preferred.

*PC skills including EPIC strongly preferred.

Responsibilities

1.Interviews patients who do not yet have follow-up appointments scheduled to ascertain clinic/location and provider of choice as applicable.

2.Schedules appointments at the correct locations or clinics. Take EPIC generated appointment letters and places in patient chart as needed.

3.Updates demographic and appointment information as needed including: documenting the patient's unit each patient's clinic name, date and time of appointment and current phone number in the admission log and updating patient's address and phone number to assure information in EPIC is the most recent demographic information.

4.Contacts clinics for early follow-up appointment as needed. If unable to schedule, schedules return visits for follow-up appointments. Makes reminder phone calls for follow-up appointed patients.

5.Collaborate with other departments to schedule follow-up retesting as needed.

Maintain positive communication with nursing staff and providers to know when patient's circumstances change which will require changing initial appointment and rescheduling.

7.Maintains accurate records of contacts and activities. Assists patients with understanding the clinic system and emphasizing the importance of attending all follow-up appointments and obtaining necessary immunizations as applicable.

Job Accountabilities

  • Identifies ways to improve work processes and improve customer satisfaction. Makes recommendations to supervisor, implements, and monitors results as appropriate in support of the overall goals of the department and Parkland.
  • Stays abreast of the latest developments, advancements, and trends in the field by attending seminars/workshops, reading professional journals, actively participating in professional organizations, and/or maintaining certification or licensure. Integrates knowledge gained into current work practices.
  • Maintains knowledge of applicable rules, regulations, policies, laws and guidelines that impact the area. Develops effective internal controls designed to promote adherence with applicable laws, accreditation agency requirements, and federal, state, and private health plans. Seeks advice and guidance as needed to ensure proper understanding.
