SUMMARY UNC Hospitals (UNCH) Department of Pharmacy has an 0.2 FTE opening for a highly motivated pharmacist seeking to join our inpatient clinical oncology team in Chapel Hill, NC. The pharmacist will gain experience in the therapeutic management of hematology, oncology, and bone marrow transplant patients through coverage across several cancer services. The individual will collaborate with physicians, APPs, nurses, and the operational inpatient pharmacy team to provide comprehensive care to a diverse patient population. Activities include multidisciplinary rounding, preparation of appropriate orders, responding to drug information requests, medication profile review, order verification, nursing support, patient counseling, and transitions of care. This individual will report to the Clinical Manager of the Hematology/Oncology Service Line and work closely with the Hematology/Oncology Clinical and Central Specialists as well as the Specialty Pharmacists within the UNC Carolina Care Pharmacy Care Network. Responsibilities: The primary responsibilities of the individual include providing comprehensive pharmacist care to patients seen on the Inpatient Hematology, Oncology, and Bone Marrow Transplant Services, collaborating closely with Outpatient Clinical Pharmacist Practitioners to ensure continuity of care between acute and ambulatory services, and leading quality improvement and outcomes initiatives. This position will ensure that clinical coverage can be provided during vacancies and support time off within the team. *Participate in daily multidisciplinary patient care rounds *Respond to drug information questions from the medical staff, nursing staff, or pharmacy colleagues *Function as a limited oncology prescribing practitioner (LOPP) to collaboratively manage chemotherapy treatment plans *Conduct medication profile review and order verification *Evaluate, monitor, and redesign medication regimens to ensure positive patient outcomes *Provide patient education *Complete Therapeutic Drug Monitoring Other Information Other information: Preferred: *Completion of a Pharmacy Practice (PGY1) Residency or equivalent experience *Completion of an Oncology (PGY2) Pharmacy Residency or equivalent experience *Board Certification in Oncology Pharmacy Licensure/Certification Requirements:* Active pharmacist licensure with the NC Board of Pharmacy or obtainable before initiation of employment. Recent graduates of ACPE-accredited colleges/schools of pharmacy may fill this role while pending initial pharmacist licensure, but are subject to limitations mandated by the NC Board of Pharmacy and must obtain licensure within 6 months of orientation or sooner if required for operations.
