Program Coordinator, Refugee Women's Health Clinic

POSITION SUMMARY:

The Refugee Women's Health Clinic (RWHC) is a program uniquely situated within the Department of OBGYN and a program within the Immigrant and Refugee Health Center (IRHC). The RWHC Program Coordinator will support the ongoing efforts of the RWHC Director in the seamless delivery of comprehensive and culturally sensitive women's health services to refugee, asylum, seeking, and recent immigrant communities in the greater Boston area. The Program Coordinator is responsible for fostering strong relationships with patients, community partners, volunteers, and healthcare providers to ensure effective and equitable programming; implementing systems to collect relevant patient data and program metrics, ensuring compliance with privacy regulations; monitoring clinic and program activities, tracking progress, and evaluating outcomes to make data-driven improvements; supporting case managers in facilitating access to and coordination among social service resources and other internal and external resources; and problem-solving with patients to both accelerate and enhance access to concrete supports and work towards obtaining patient autonomy. They will support members of the case management and behavioral health teams in triaging care appropriately; work with patients to identify strengths and barriers; facilitate referrals to appropriate medical/clinical providers and social services and resources; and create a nurturing warm, welcoming environment for new patients. The Program Coordinator will be responsible for communicating with partner agencies, including the Department of Public Health, as well as data entry and record keeping related to DPH initiatives.

The Program Coordinator will comply with all data collection, data entry and data integrity needs both for facilitation of clinical care and reporting requirements to the BMC health system and funders. The individual will collaborate with clinical staff, administrative staff and research personnel in RWHC and will partner closely with IRHC front desk staff to appropriately welcome, evaluate and triage patients. Additionally, the Program Coordinator will be responsible for administrative tasks including maintaining supplies, coordinating with department staff to reserve spaces, and building Epic templates for staff and trainees.

ESSENTIAL RESPONSIBILITIES / DUTIES:

Administrative and program development responsibilities


  • Implements systems to collect relevant patient data and program metrics, ensuring compliance with privacy regulations
  • Develops and maintains strong relationships with the community and community resources to ensure patient access
  • Monitors clinic and program activities, tracking progress, and evaluating outcomes to make data-driven improvements
  • Participates in multidisciplinary/staff/team meetings and activities; meets regularly with leaders with IRHC, OBGYN, and Care Management Social Work to triage program issues appropriately.
  • Participates in team-based Quality Improvement (QI) initiatives
  • Participates in local site operations, including team operational meetings; and is actively engaged in developing, implementing and refining operational protocols and processes as IRHC infrastructure expands
  • Communicates with partner agencies, including the Department of Public Health, as well as data entry and record keeping related to DPH initiatives.

Support for newly arrived unhoused pregnant patients and families


  • Support case management services for newly arrived immigrants and existing RWHC clients, including connecting patients to a broad range of services including but not limited to: mental health services, primary care services, medical specialty services, immigration legal services, financial assistance, food, transportation, housing, clothing, health literacy, interpreter service needs, etc.
  • Create a safe, warm, welcoming environment and encourage engagement in care
  • Coordinate with OB Triage, OBGYN Ambulatory Clinic, Community Health Centers and other care providers in the area to provide access to timely prenatal care for pregnant arrivals
  • Work across the city and state to coordinate care at the most appropriate site for patients and their families near their shelter placement or within the community
  • Collaborate with IRHC Case Management Team, OB Social Work and Pediatric ED Social Work
  • Function as a key member of an interdisciplinary team (nurse, social workers, behavioral health clinicians, physicians, resource specialists, clinical support staff, etc.)
  • Provide client education regarding available services and resources both within the IRHC, throughout BMC, across the area and state as needed and provide tools and facilitate connections to these resources
  • Maintain HIPAA standards and confidentiality of protected health information.
  • Perform and maintain all documentation in the electronic chart of patient encounters; resource development efforts; and other patient interventions and interactions

JOB REQUIREMENTS

EDUCATION:

Masters' degree in social work (MSW), public health (MPH) or counseling preferred, or Bachelor's degree in social work, public health or related human services field with minimum 3 years relevant experience.

CERTIFICATES, LICENSES, REGISTRATIONS REQUIRED:

EXPERIENCE:


  • Minimum of three years of relevant experience
  • Experience with mental health populations preferred;
  • Experience with diverse immigrant and refugee populations preferred, including lived experience as immigrant to the United States.
  • Experience and comfort with risk assessment and appropriate triaging preferred
  • Comfortable innovating in information technology systems; experience using/documenting in electronic medical record systems
  • Experience in motivational interviewing is a plus
  • Preference for candidates who are bilingual in English and Haitian Creole, Spanish, Portuguese, Somali, Arabic, or French.
