ACT Case Manager

Our Assertive Community Treatment Team is looking for a Case Manager who has experience working in the substance use and mental health field. You will be an integral part of our multidisciplinary team to help people avoid crisis situations and to keep crises from turning into unnecessary hospitalizations. ACT is designed to help those individuals who are struggling with serious and persistent mental illness and co-occurring substance use disorders that may need more than the traditional outpatient mental health care and psychiatric rehabilitation more intensive services. As the Case Manager will provide comprehensive community support to those individuals living in their homes. Your role will be to help stabilize individuals by assessing and evaluating all clients on an ongoing basis, provide support services and/or crisis intervention as needed, develop, design and implement treatment goals/plan with the client to promote the individual's recovery. In addition, collaborate with community resources while promoting recovery, improving their mental health and rehabilitation. This is a full time position with benefits.

Education/Licensing Requirements:


  • BA/BSW in a human services related field required.
  • Knowledge of substance abuse and mental health is a plus.
  • Experience with Electronic Health Record systems is preferred.
  • Approved driver's record and reliable transportation required.
