Job Roles

  • In a support capacity, build positive relationships, inspire others to perform at a higher level, and assist on career developmental plans in retaining an accountable workforce.
  • Provide input on appropriate disciplinary action on staff supervised in a timely manner.
  • Provide supervision, guidance, and training to the Home Care staff.
  • Ensure staff receive all new hire and annual trainings on time.
  • Ensure the participant-centered plan of care is followed.
  • Schedule staff for home visits and work assignments.
  • Coordinate home care delivery and implementation of all home care services that may include personal care services to ensure quality delivery of personal care services as outlined in plan of care.
  • Create and maintain inventory of all home care and durable medical supplies.
  • Regularly complete home visits to evaluate home care needs, and facilitate home care services to ensure quality of care for each PACE participant.
  • Communicate and interact with PACE participant's caregivers and/or family members.
  • Ensure the highest quality of care is provided and is in compliance with regulatory requirements, and departmental policies and procedures.
  • Act in the role of liaison between the interdisciplinary team, caregiver representatives and community agencies.
  • Provide ongoing education and/or instructions to the PACE participant, family, and/or caregivers about goals and expectations, safety at home, use of equipment and other topics as indicated, and as allowable under the LVN license.
  • Participate as a member of the Interdisciplinary Team through attendance at staff meetings, take part in the participant care planning, and communicate any changes in participant condition to team members.
  • Conduct staff competencies, evaluations, education and guidance and provide information to manager.
  • Participate and assist in the development and implementation of policies and procedures.

Education/Certifications/Licenses/Registrations

  • Current California LVNLicense.
  • Current American Heart Association healthcare provider CPR (BLS), or Advanced Cardiac Life Support, Advanced Trauma Life Support, or Advance Resuscitation Training.
  • Ability and means to travel as needed in a timely manner within San Diego County, to locations that may have limited access to public transportation; proof of liability and property damage insurance on vehicle used is required. DRIVER REQUIREMENTS: Age 21; Licensed for a minimum of 3 years; No violations and/or accidents within 3 years; No vehicle related suspensions/reinstatements; No DUI, reckless or felony Driving within 7 years.

Experience/Specialized skills (including Language)

  • 1 - 2 years of experience working with the older adult population in a leadership role.
  • Clinical competency with the older adult population preferred.
  • Ability to work effectively in a team environment.
  • Demonstrated ability to communicate effectively and openly with other interdisciplinary team members on participant status and needs.
  • Must be able to multi-task, be flexible, ensure accuracy, and meet changing priorities in a fast-paced, high workload environment.
  • Basic computer literacy to comply with department needs and expectations (i.e., E-HR documentation, obtaining background information and reports, following up on appointments, etc.).
  • Bilingual English/Spanish preferred.

25.74 - 36.81

If you are hired at Family Health Centers of San Diego, your final base salary compensation will be determined based on factors such as geographic location, jurisdictional requirements, skills, education, and/or experience. In addition to these factors - we believe in the importance of pay equity and consider internal equity of our current team members as a part of any final offer. Please keep in mind that the range mentioned above is what we reasonably expect to pay for the role. Hiring at the maximum of the range would not be typical in order to allow for future and continued salary growth.
