City/State Harrisonburg, VA Overview Work Shift First (Days) (United States of America) Sentara Rockingham Memorial Hospital is hiring a Community Health Worker! As a Community Health Worker you will work in collaboration with the Case Manager and other healthcare team members to provide comprehensive and coordinated care. Engages in interventions to improve self-management skills and improve quality of life for patients with chronic and/or complex illness. Provides education and behavior modification strategies and provides social support to patients and families. Acts as a patient advocate and healthcare navigator in assisting with resource allocation and management. Requirements- Bilingual in English and Spanish is preferred. Experience working in a community-based setting preferred. Knowledge of medical terminology preferred High School Diploma or equivalent- Required, Drivers License with a clean driving record- required. Facility Spotlight- Sentara RMH Medical Center, a Magnet designated hospital located in Harrisonburg VA, serves a seven-county area with a population of 218,000 residents.
