THE POSITION

Westmoreland County Area Agency on Aging is seeking an experienced Aging Care Manager 2 to join our team. In this position, you will have the opportunity to play a vital role in providing high-quality care and support to elderly individuals throughout the community. DESCRIPTION OF WORK

In this role, you will be providing information and referral services at the Agency's office site via telephone, computer, fax, mail, and office visits. Work includes obtaining pertinent information through direct contact with consumers, members of the consumers' families, professionals, and other persons necessary in order to evaluate the information and to determine consumer needs and the appropriate course of action. You will also be providing informational and educational programs to senior groups, civic clubs, or any interested organizations in Westmoreland County. You will have the opportunity to provide in-service training for home health agencies, subcontractors, and other social service agencies upon request, as well as set up and staff agency exhibits at health fairs, shopping malls, and other public service events. Lastly, you will be providing information on services and making referrals by completing intake forms and screening tools.

Work Schedule and Additional Information:

  • Full-time employment
  • Work hours are 8:30 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch.
  • This position may require standby rotation for off-hour coverage.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS

Minimum Experience and Training Requirements:

  • Six months as an Aging Care Manager 1 or a County Caseworker 1; or
  • Successful completion of the County Social Casework Intern program; or
  • Six months of professional human services or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or
  • An equivalent combination of experience and training.

  • Hiring preference for this vacancy may be given to candidates who live within Westmoreland, Allegheny, Armstrong, Cambria, Fayette, Somerset, Indiana, or Washington counties. If no eligible candidates who live within these counties apply for this position, candidates who reside in other counties may be considered.
  • You must be able to perform essential job functions.
