JOB PURPOSE:

The Case Manager will provide comprehensive case management to clients and their families who are homeless or at risk of homelessness with the goal of increasing their quality of life and maintaining the highest possible ability to function within the community.


Qualifications

ESSENTIAL JOB RESPONSIBILITIES:



  • Advocate for and facilitate clients' access to community resources, housing assistance, utility assistance, and relevant community services and benefits. Build database of community resources. Provide referrals to community resources as needed.
  • Update Client Files in HMIS (Homeless Management Information System) and Sales Force.
  • Conduct initial intake meeting with the client(s). Evaluate individual and family needs. Complete service plan with the client including housing and other related needs. Coordinate and monitor services, including comprehensive tracking of client activities in relation to service plan and Housing Inspections. Document detailed case notes, daily; maintain comprehensive client files. Prepare reports as requested by Lead Case Manager, Program Manager and/or Quality Assurance.
  • Meet regularly with Lead Case Manager to staff case load. Provide on-going program evaluation and recommendations to the Lead Case Manager and Program Director for continuous growth and quality.
  • Be active in and network at monthly community groups, events, and meetings. Provide presentations at various community groups, events, and meetings within areas of responsibility to include rural and metropolitan areas.
  • Other duties as assigned.

ESSENTIAL QUALIFICATIONS:

EDUCATION: Bachelor's Degree in Social Work, Sociology, Psychology or related field, preferred but not required.

EXPERIENCE: 1+ years case management experience; 3+ years preferred. 2+ years in a customer service focused environment. Experience with homeless, low income, individuals & their families a strong plus.

ATTENDANCE: Must maintain regular and acceptable attendance at such level as is determined in the employer's sole discretion.

LICENSES: Driver's License with clear record.

VEHICLE: Must have daily use of a vehicle without prior notice.

OTHER: Must be available and willing to travel to various locations and with such frequency as the business need dictates.
