JOB PURPOSE: The Intake/Health and Wellness Coordinator plays a vital role in our organization, responsible for enrolling, facilitating, and implementing comprehensive programs and activities aligned with Endeavor's six principles of Wellness. Additionally, they conduct initial eligibility screenings for individuals and family members seeking program assistance. For those who may not meet eligibility requirements, the coordinator provides general information about internal services and community referrals. This position is dedicated to ensuring access to care for all enrolled individuals in the program with empathy, compassion, and a commitment to promoting wellness and holistic care. Qualifications
ESSENTIAL JOB RESPONSIBILITIES:
ESSENTIAL QUALIFICATIONS: EDUCATION: Bachelor's degree, in Public Health, Social Work, Health Education, or related field required. Strong preferences for National Wellness Institute's Certified Wellness Practitioner (CWP) or similar certification EXPERIENCE: Experience working with a Military, Veteran, or Law Enforcement population is strongly preferred. Must demonstrate leadership capacity and ability to foster team-building and a supportive, flexible environment. ATTENDANCE: Must maintain regular and acceptable attendance at such level as is determined in the employer's sole discretion. LICENSES: Driver's License with clear record. Strong preference for National Wellness Institute's Certified Wellness Practitioner (CWP) or similar certification. VEHICLE: Must have daily use of a vehicle without prior notice. OTHER: Must be available and willing to travel to various locations and with such frequency as the business need dictates.
