JOB PURPOSE:

The Intake/Health and Wellness Coordinator plays a vital role in our organization, responsible for enrolling, facilitating, and implementing comprehensive programs and activities aligned with Endeavor's six principles of Wellness. Additionally, they conduct initial eligibility screenings for individuals and family members seeking program assistance. For those who may not meet eligibility requirements, the coordinator provides general information about internal services and community referrals. This position is dedicated to ensuring access to care for all enrolled individuals in the program with empathy, compassion, and a commitment to promoting wellness and holistic care.


Qualifications

ESSENTIAL JOB RESPONSIBILITIES:



  • Develop and oversee wellness programming consistent with the Endeavor's Wellness Model for Veterans and/or Law Enforcement personnel.
  • Promotes wellness initiatives; answers question and collects feedback from participants.
  • Collaborates with leadership to identify cost-effective wellness programs and incentives.
  • In coordination with an integrated care team, assesses and analyzes the client population's general physical and mental health needs.
  • Set up initial client file for clients and their families entering the program.
  • Collaboration with external entities for support, events, opportunities, education, etc.
  • Support planning and coordination of the program and its activities.
  • Set up initial client file for clients and their families entering the program.
  • Assign clients to needed assistance (wellness, fitness, clinician, other services etc.), track clients progress through the program.
  • Prepare and keep data, files, reports, drafts and accurate reporting of intake/referral/list activities.
  • Communicate with team members to ensure maximum efficiency.
  • Manages daily assignments and ensures deadlines are met and work is completed correctly
  • Monitor the progress of the client concurrent with the program and adjust as necessary to ensure the successful completion of intakes/outreach efforts, weekly brief with Director of Wellness
  • Designs, implements, and executes plans/programs such as meditation classes, support groups (men's, women's, parents), life skills classes, and other activities to support client needs.
  • Support Wellness Program Manager in coordinating classes, speakers, seminars, training, and voluntary assessments to promote healthy lifestyles.
  • Assist the Program Manager in monitoring the programs referrals, intakes and wait list and report on the programs predetermined objectives.
  • Drafts and maintains accurate, confidential records and reports as required to track and assess client participation, program incentives, costs, and benefits to the company.
  • Other duties as assigned.

ESSENTIAL QUALIFICATIONS:

EDUCATION: Bachelor's degree, in Public Health, Social Work, Health Education, or related field required. Strong preferences for National Wellness Institute's Certified Wellness Practitioner (CWP) or similar certification

EXPERIENCE: Experience working with a Military, Veteran, or Law Enforcement population is strongly preferred. Must demonstrate leadership capacity and ability to foster team-building and a supportive, flexible environment.

ATTENDANCE: Must maintain regular and acceptable attendance at such level as is determined in the employer's sole discretion.

LICENSES: Driver's License with clear record. Strong preference for National Wellness Institute's Certified Wellness Practitioner (CWP) or similar certification.

VEHICLE: Must have daily use of a vehicle without prior notice.

OTHER: Must be available and willing to travel to various locations and with such frequency as the business need dictates.
