Summary

Pathway Home, a DHS funded Interim Housing - Encampment Resolution program, is a housing-focused solution to combating homelessness focused on encampment locations in Incorporated and Unincorporated areas of Los Angeles County. TPC provides high quality and customized services to individuals experiencing homelessness, utilizing a Trauma Informed lens.

Essential Duties and Responsibilities:




  • Conduct initial health assessments of new admissions to identify immediate health concerns and ongoing medical needs.
  • Provide wound care, first aid, and other basic medical treatments as needed (within scope of practice).
  • Coordinate PCP appointments or other medical follow ups upon admission.
  • Provide medical case management and supportive services to individual members (caseload of 20-35).
  • Provide wound care to members as needed.
  • Document and chart all services provided on nurse charting form with accuracy and in real time.
  • Provide medication management to individuals including but not limited to medication monitoring, patient teaching, and supportive services to increase compliance with medication regimens.
  • Work in collaboration with the rest of the Palm Motel team- inform site leadership, case managers, and client's care team of any medical and / or medication concerns.
  • Maintain medication logs and medication storage in compliance with safety standards and industry standards of care.
  • Participate in regular staff meetings and trainings to stay updated on program developments and clients' progress
  • Be able to notify the appropriate community agency in an emergency (i.e., police, paramedics, fire department).
  • Maintain open communication with direct supervisor, individual and group supervisor(s).
  • Ensure that medical services are reflective of the agency's treatment model, which emphasizes member choice, harm reduction, trauma-informed care, and psycho-social rehabilitation.
  • Maintain accurate and confidential health records per HIPAA and program guidelines, document all care provided and significant interactions.
  • Conduct Health/Wellness and medication management groups.
  • Conduct Health/Well checks daily
  • Other duties as assigned.



Qualifications:




  • Current, California license as a Licensed Vocational Nurse with a clear license.
  • Minimum one years' experience working with individuals experiencing chronic homelessness, mental health, and substance abuse issues.
  • Able to physically lift, move, and position members as needed.
  • Able to demonstrate acceptance toward members who may have poor coping skills and a high degree of cultural sensitivity to their needs.
  • Experience completing medication logs to accepted industry standards.
  • Proficient in Microsoft Word, Outlook, Excel, and database applications.
  • Detail oriented with excellent time management, organizational, written, verbal, interpersonal, and computer skills.
  • Able to learn quickly and work effectively with a wide range of constituencies and with minimal supervision.
  • Valid CA Driver's license, reliable car, auto insurance, and an acceptable driving record.
  • Able to obtain and maintain CPR/1stAid certification.



Preferred Qualifications:




  • Experience working with those living with mental illness and/or substance addictions.
  • CPR certification and additional training in emergency medical response preferred
  • Experience completing DHS documentation.
  • Experience with HMIS



Work Environment:




  • Combination of field and office environment
  • Regularly required to sit, stand, bend; occasionally lift or carry up to 35 lbs.
  • On occasion walk or drive to different local sites
  • Will necessitate working in busy and loud environments
  • Will be exposed to elements like cold, heat, dust, noise and odor
  • May need to bend, stoop, twist and sit throughout the day
