Job Overview:

Responsible for overall care coordination of patient and family from admission through discharge. Guides family through continuum acting as liaison between family, multidisciplinary team, physicians and external agencies. Monitors family and patient's understanding and compliance with treatment and discharge goals and discharge goals and coordinates team functions toward achievement of discharge plan. Maintains contacts with third party to assure ongoing funding of care and maximum utilization of services available to assure child's ongoing care. *This is a 6-month temporary position*

Qualifications:

Required:

  • Bachelor s Degree in Nursing, Social Work or related Allied Health Care field
  • Meets the accrediting and|or licensing standards of the profession
  • AHA BLS Provider Certification
  • Minimum of three years' experience in pediatric setting. (For MS, MSW minimum of one-year experience in pediatrics)
  • Excellent communication skills both verbal and written
  • Excellent interpersonal skills
  • Good planning and organizational skills

Preferred:

  • Master s Degree

Certifications and Licenses Required:

  • Meets the accrediting and|or licensing standards of the profession
  • AHA BLS Provider Certification

Scheduling Requirements:

  • Temporary position, 6 months
  • Full Time (40/hours/week)

Essential Functions:

  • Effectively engages individuals/families in helping relationships using acquired professional skills
  • Demonstrates understanding of patient's medical diagnoses, prognosis, progress and implications for family support, education and discharge planning
  • Transmits clinical information accurately to third party payers and community agencies
  • Engages in advocacy for patient, and foster family's abilities to assume independent skills in seeking and obtaining supports and services for child
  • Acts to enhance understanding of CSH services and mission through development of relationships with community and government agencies, third party payors, service providers and referral sources
  • Demonstrates understanding of health care system, managed care and resource utilization as well as policies and services available for patient populations
  • Effectively communicates financial and social needs/barriers/issues to the multidisciplinary team, physicians and external agencies
  • Identifies families at high risk for child neglect or abuse and makes appropriate referrals
  • Completes proper documentation in a timely manner. Provides necessary documentation to external agencies as needed
  • Participates in projects for the department that are consistent with department and hospital goals and missions
  • A high degree of customer sensitivity is required. Excellent cognitive analytical and problem-solving skills essential. Must be able to manage multiple tasks simultaneously
  • Occasional visits to other CSH sites, referring and receiving hospitals, and external agencies may be necessary
