Transplant Coordinator 2 (TRANSPLANT CRD 2)-Pediatric Cancer Center

Transplant Coordinators in the Pediatric Bone Marrow Transplant department at UC Davis Health provide care to pediatric and young adults with a variety of medical and surgical needs. The Pediatric Transplant Coordinator 2 is a nursing resource in the care of the pre & post-transplant patient, with a focus in autogolous, allogeneic, and CAR-t transplants. The Pediatric TC 2 has responsibility for clinical case management of pediatric/young adult stem cell transplant candidates from the time of referral until transplant. The Pediatric TC 2 will also provide post-transplant care/case management. The Pediatric TC 2 may also assist with the case management care of other pediatric hematology/oncology diagnoses; such as leukemia, lymphoma, neuroblastoma, etc. These responsibilities involves coordination of the interdisciplinary medical care delivery system under the direction of the Medical Director. The Pediatric TC II functions as the liaison between the transplant team and the patient, family, medical staff, referring physicians, donor agencies, and insurance companies. The Pediatric TC 2 applies advanced patient assessment skills and knowledge in the area of pediatric hematology/oncology/BMT, and performs a wide range of duties as allowed by the Nurse Practice Act. The Pediatric TC 2 is accountable for their own practice while providing direct patient care to a diversified patient population.

Applies knowledge, experience and judgment to determine the importance of a situation, set priorities, and use abstract thought to evaluate clinical situations. Participates in patient and staff education. Demonstrates leadership skills. Demonstrates accountability for own practices as defined by the Nurse Practice Act.

Minimum Qualifications

  • Graduation from an accredited school of nursing.
  • Three (3) years of recent Pediatric Bone Marrow/Stem Cell Transplant Experience or Three (3) years of recent Adult Bone Marrow/Stem Cell Transplant Experience.
  • American Heart Association (AHA) Basic Life Support (BLS) or UC Davis Health CPR.
  • Valid California Registered Nurse License without restriction
  • Ability to read, write, speak, and understand the English language.
  • Ability to utilize medical terminology appropriately.
  • Knowledge of current trends in nursing.
  • Commitment to excellence in patient care.
  • Effective interpersonal skills.
  • Demonstrates accountability and professionalism.
  • Adapts to change in a positive, professional and productive manner.
  • Knowledge of legal implications in clinical practice and documentation.
  • Knowledge of performance improvement and evidence-based practice.
  • Basic computer skills.
  • Ability to assess, plan, implement and evaluate patient care, taking into consideration protective interventions, development (behavioral and physical) and physiological characteristics specific to the patients.

Preferred Qualifications

  • Bachelor of Science in Nursing (BSN) preferred.
  • Outpatient Case Management experience
  • Telephone Triage experience.
  • Surgical Care Coordination experience
  • Ambulatory experience
  • Certified Pediatric Hematology Oncology Nurse (CPHON), Oncology Nurse Certification (OCN), or Blood and Marrow Transplant Certified Nurse (BMTCN)
