Your Role With Us:MSW Licensed Care ManagerUnder the general supervision of the Palliative Care Medical Director and the Clinic Manager, the Palliative Care Social Worker will provide evidenced-based brief solution-focused psychosocial interventions to support the collaborative process of biopsychosocial assessment and treatment, advanced care planning, goals of care discussions, and support for coordination of care to meet a patient's psychosocial and health care needs. This position is part of the Palliative Care Team and shares responsibility for establishing relationship with patient, family, providers, and clinic staffCore Functions: Completes a comprehensive Psychosocial Assessment intake for assigned patients.Coordinates with the multi-disciplinary treatment team of the department and the unit for coordination of interventions and services.Participates in care conferences in partnership with palliative care physicians and other designated team members.Facilitates patient/family understanding, decision making, and capacity to comply with medical, psychiatric, dual diagnosis and behavioral recommendations.Documents social work interventions and patient contacts in the patient record in a clear, concise, and timely manner.Reports mandated child and elder abuse/neglect as required by Oregon law.
