POSITION SUMMARY:
A Care Services Coordinator is a navigator and resource for families living with ALS. A Care Services Coordinator assists those diagnosed with ALS and their caregivers in attaining their maximum functional potential. Acts as a liaison between patient/family, healthcare personnel, and community resources to ensure necessary care is provided promptly and effectively. This position reports to the Care Services Director and will be based in Nashville, Tennessee. DUTIES AND RESPONSIBILITIES: Program Delivery and Measurement - Performs routine to moderately complex care management activities and patient advocacy across the continuum of care in order to facilitate and promote quality of life and minimize fragmentation of health care delivery. Manages an assigned caseload of Nashville clients to include, but not limited to, developing care plans, home assessment visits, ALS clinic support, outreach and enrollment of clients.
Support Groups - Plans and facilitates monthly support group(s) for clients/families; occasionally provide educational portion of monthly meeting; arrange for professional/expert topic speakers. Responsible for growth and function of support group(s). Administrative Functions - Ensures that all materials provided and utilized are in accordance with National or Chapter policy and best practices; maintains confidential document and computer-based files of clients' information and retrieval system for interrelated files and records; prepares regular reports of activities; ability to format and proofread detail-oriented letters, memoranda, and reports using Microsoft Office programs.
QUALIFICATIONS:
PAY TRANSPARENCY: The ALS Association's pay range for this positionis $22.51 - $27.99 per hour. The ALS Association has compensation ranges for various work locations throughout the United States, allowing us to compensate employees competitively and consistently in diverse geographic markets. The above-listed listed range shows prospective compensation for this role; the appropriate range will be established by what is eventually determined to be the candidate's primary work location. Various factors, including the complexity and significance of the role, job duties/requirements, relevant experience and abilities, and primary work location, determine individual compensation. Compensation ranges are evaluated and typically adjusted on a regular basis. Offers are made within the compensation range that is in effect at the time of the job offer. The Association may, at its discretion, offer merit-based increases tied to individual performance. The benefits listed may differ depending on employment status with the Association.
