Care Coordinator - Intensive Care Management UnityPoint Health - Black Hawk Grundy Mental Health Center Shift: Full-time; M-F; Days Benefits Eligible Sign-On Bonus Eligible (for external new hires only) - $5,000 paid out over 1 year Responds to the needs of adults receiving Habilitation Services or Children on the Child Mental Health Waiver. This position will provide whole-person care coordination across medical, behavioral, and social service community systems while ensuring all habilitation and child mental health waiver rules are met. Responsibilities
- Perform Annual assessments for habilitation or child mental health waiver services as required by Iowa Code, Iowa Medicaid Enterprise, and Managed Care Company.
- Complete Home and Community Base Services community housing assessments annually.
- Generate social histories as required by Iowa Code.
- Guide members to establish person centered goals in order for them to be successful living in the community through assessment and planning completed by the Intensive Care Manager.
- Develop person centered treatment plans per Iowa Code guidelines.
- Request service authorizations from Iowa Medicaid Enterprise or managed care companies for habilitation or child mental health waiver services.
- Evaluate if services are being furnished in accordance with the member's service plan, including the amount of service provided and the member's attendance and participation in the service.
- Determine if services in the service plan are adequate, including the member's progress toward achieving the goals and actions determined in the treatment plan.
- Appraise changes in the needs or status of the member, make necessary adjustments in the service plan and service arrangements with the providers. Conduct home visits/face to face visits and contact members by phone as required by Iowa Code.
- Document home visits, face to face visits, phone calls and collateral contacts in Electronic Health Record.
- Track due dates for assessments, social histories and treatment plans. Adhere to time sensitive deadlines to ensure members services do not lapse.
- Address treatment barriers for each low to moderate risk patient. Refer high risk patients to Nurse Care Manager.
- Promote patient's health and ensure that all personal health goals are included in person-centered care management plans.
- Facilitate meetings, staffing, care plan conferences, etc. related to enrolled members.
- Establish relationships with habilitation and child mental health waiver service providers.
- Maintain relationships with Managed Care Company and Iowa Medicaid Enterprise.
- Documentation of services using the Center's Electronic Health Record and other Provider Portals as required.
- Maintain patient confidence and protect operations by keeping information confidential.
- Participation in meeting performance measures and continuous quality improvement activities, including data collection, tracking, and analysis.
- Adhere to all policies and procedures set forth by UnityPoint Health and other funding sources.
- Accomplish all tasks as appropriately assigned or requested.
Qualifications
Minimum Requirements:
- Bachelor's Degree in a Human Service-related field
- One-year experience working with adults and/or children with mental illness.
- Valid driver's license
- Write read, comprehend, and speak fluent English
- Demonstrate excellent customer service and collaborate with physicians, members, visitors, staff, and the health care community.
Preferred:
- Two to four years' experience working with adults and/or children with mental illness.
- Knowledge of habilitation services and child mental health waiver services.
- Knowledge of community resources.
