WHAT YOU WILL BE DOING (ESSENTIAL DUTIES) ...

The SOAR (SSI/SSDI Outreach, Access, and Recovery) Case Manager plays a crucial role in assisting clients with applying for or reinstating state-supported insurance to facilitate access to community-based behavioral health treatment.

Essential Duties:

  • Assist clients in navigating and applying for state-supported insurance programs and ensure compliance with relevant federal/state laws and regulations during the application process.
  • Collaborate with healthcare providers and community resources to facilitate access to behavioral health treatments.
  • Maintain accurate and confidential client records in accordance with HIPAA and other regulatory standards.
  • Identify applicants through team meetings, outreach, and referrals from referral sources and community partners.
  • Conduct interviews to gather necessary information for SSI/SSDI applications.
  • Collect medical records and other required documentation for SSI/SSDI applications.
  • Write SOAR Medical Summary Reports for individual applications
  • Accompany individuals to appointments at the Social Security Administration
  • Coordinate visits to medical doctors, psychiatrists, and other specialists to obtain evidence for the application.
  • Coordinate case management services with partners and provide direct case management support as needed.
  • Assist the team with administrative tasks when needed.

WHAT WE EXPECT OF YOU...

  • Bachelor's degree preferred in public health, health education, or a related field.
  • Minimum of two years of experience in benefits administration, insurance coordination, or a similar role within a healthcare or social services setting.
  • SOAR certification is preferred. If not currently SOAR certified, NMH will pay for the training and certification
  • Solid understanding of public and private benefits systems and state resources.
  • Familiarity with federal/state laws, accreditation standards, and regulations governing health care record confidentiality.
  • Exceptional interpersonal and communication skills, both verbal and written, with the ability to interact professionally with diverse individuals.
  • Compassionate, empathetic, and dedicated to delivering high-quality customer service.
  • Self-motivated and capable of working independently, with strong organizational skills and the ability to manage priorities to meet deadlines.
  • Possession of a valid Driver's License and reliable transportation.
