RFK Community Alliance has an exciting opportunity for a part-time Adult Services Nurse to join our team in Lancaster!
Who We Seek:
We are looking for an experienced nurse to join our exceptional nursing team. The nurse will work as a part of the Adult Services team to provide supervision of medication management and health coordination for our adult residential and day population. This position is approximately 30 hours per week and can be somewhat flexible.
Responsibilities Include:


  • Approach colleagues and all job-related activities from a perspective that recognizes and honors diversity, equity, and inclusion.
  • Assist in maintaining health records.
  • Triage of injury, illness or accident and provide appropriate intervention.
  • Provide the family with support, counseling, and advocacy as it relates to resident health needs and long-term planning.
  • As part of the admission process, conduct the assessment of the new resident, provide information about the services of the program, and review pre-admission records to ensure that all medical information is present.
  • Coordinates and document discharge planning, transition, and follow-up services.
  • Participates in program meetings as a member of the Operations Team.
  • Maintain interdisciplinary contact with residential staff and the coordinators.
  • Reviews the Ehana Health Records System regularly and documents pertinent resident medical needs and documents nursing progress notes within the record.
  • Provide on-call for the Adult Services programs for medical needs.


Requirements:


  • Valid license to practice as a Licensed Practical Nurse in Massachusetts
  • A minimum of two years of employment as a Nurse in a residential health, community health, or other relevant clinical nursing setting


Hours: Monday-Friday 30-40hr (Flexible)
