Job Summary

Plans and implements specific treatment programs for individual patients according to principles and practices of physical therapy. Collaborates with an interdisciplinary team in the development of the patient's individualized plan of care. Provides direction and supervision to Physical Therapy Assistants, Physical Therapy students, and Certified Nursing Assistants in the area of patient care. Provides program and staff development support.
An advanced level of clinical and interpersonal skills are required.

Accountabilities

  • Assessment:

    - Performs appropriate initial physical therapy assessments and reassessments within the

    time frames required by the Agency.

    - Incorporates appropriate objective tests and measures to facilitate outcome

    measurement.

    - Uses Agency assessments and screening tools as a part of the assessment process

    ex) high risk for rehospitatlization tool, falls assessment tool, nutritional screening,

    screening for abuse and neglect, Oxygen safety, spiritual assessment...

    - Recognizes the need for referrals to other Home Health disciplines as appropriate and

    contacts the patient's physician with recommendations to maximize patient outcomes.

    - Demonstrates advanced knowledge of diagnoses / conditions seen in physical therapy.

    - Identifies actual or potential problems that would impact the PT treatment plan as well as

    the overall patient plan of care (such as cognitive deficits, family situations, etc.)

    - Documents assessment findings, including OASIS assessment responses, accurately and

    in the timeframes established by CMS and Agency policy using only Agency approved

    terminology and abbreviations.
  • Planning:

    - In collaboration with the patient's physician, establishes the plan of care for the individual

    patient based on the results of the patient assessment.

    - Reviews available documents from the referral source that may assist in the development

    of the patient plan of care ie: acute care discharge summary, rehabilitation notes.

    - Sets realistic goals and expected functional outcomes.

    - Designs/redesigns the treatment plan as appropriate for the patient's age, educational

    level and cultural background

    - Designs a plan of intervention that is coordinated and integrated with other services the

    patient is receiving.

    - Actively encourages the patient and caregivers to participate in development of the plan of

    care.

    - Establishes specific treatment goals based on the results of the evaluations. Long-term

    and short-term goals are appropriate, achievable and measurable within a specified time

    frame.

    - Documents the plan of care timely according to Agency policies.

    - Plans for the discharge of the patient taking into consideration other disciplines, goal

    achievement and patient outcomes.

    - Based upon reassessment findings and the patient's response to treatment and goal

    achievement, redefines problems and modifies goals and treatments as indicated by the

    interdisciplinary treatment plan

    - Provides the patient and caregiver with information regarding follow-up services which

    may include a referral to other community resources.
  • Intervention:

    - Provides treatment according to the established frequency and duration.

    - Demonstrates awareness and use of proper safety measures regarding the patients,

    others in the home and self.

    - Demonstrates good working knowledge of various Physical Therapy techniques with

    flexibility and creativity in the implementation of these therapy techniques

    - Interacts as appropriate with others involved in the patient's care to incorporate an

    interdisciplinary treatment approach for coordinated quality care.

    - Provides developmentally appropriate education and training to patients, families and

    caregivers regarding diagnoses, progress and long-term functional outcomes.

    - Documents interventions in a timely manner using only Agency approved terminology and

    abbreviations.
  • Evaluation:

    - Evaluates the plan of care for effectivenss and determines progress toward expected

    outcomes throughout the patient's episode of care.

    - Consults with team members and leadership for complex situations and when problems

    are unresolved. Arranges for a conference with team members, leadership, patients, and

    caregivers as appropriate.

    - Discharges the patient when appropriate. Prepares the patient throughout the episode for

    discharge. Obtains written notice of discharge in accordance with the Medicare Conditions

    of Participation.

    - Evaluates individual practice through comparison of the individual therapist's patient

    outcomes (caseload) with state and national benchmark data for patient outcomes.
  • Supervision:

    - Performs supervision for physical therapy assistants in accordance with the SC Physical

    Therapy Practice Act.

    - Supervises the development of Physical Therapy students to enhance the learning

    experience for the student.

    - Exercises sound judgement when delegating tasks to Physical Therapy Assistants and

    Physical Therapy students.

    - Performs supervisory visits for Certified Nursing Assistants in accordance with Medicare

    Conditions of Participation.

    - Provides feedback to assistants and students in a professional manner.

    - Documents supervision and supervisory visits as per State and Agency policies in a timely

    manner.
  • Productivity

    - Meets the team / department productivity standards

    - Complies with established standards of efficient patient scheduling to maintain

    appropriate levels of productivity

    - Assists with additional coverage when caseload permits and when multiple caseloads

    may be modified easily to permit one person to assist with coverage of another area

    - Accepts and examines performance feedback and incorporates this into their conduct.
  • Department and Team Responsibilities

    - Completes PPD testing procedures by the end of the employee's birth month

    - Maintains established departmental policies and procedures, objectives, quality

    assurance programs, safety, environmental and infection control standards

    - Attends staff meetings, team meetings and mandatory inservices as scheduled

    - Completes assignments in a timely and thorough manner

    - Assists with care and maintenance of departmental equipment and supplies

    - Performs other related duties as assigned or requested

    - Completes mandatory training annually and in a timely manner

    - Demonstrates willingness to assist and is respectful of the rights of other team members.

    - Assist with the orientation and ongoing development of new Agency staff members as

    needed.

    - Promotes healthcare in the community through education and assistance at community

    events.

Supervisory/Management Responsibility

This is a non-management job that will report to a supervisor, manager, director, or executive.

Minimum Requirements

  • Bachelor's Degree from an APTA Accredited Program
  • 1 year of Physical Therapy experience

Required Certifications/Registrations/Licenses

  • PHYSICAL THERAPIST-Licensed or eligible for licensure in the State of South Carolina (Lawson Code CLPT)
  • Valid South Carolina driver's license (Lawson Code LSCD)
  • "You can use a valid driver's license issued by the state from which you moved for 90 days, after which time you must surrender it and obtain a South Carolina driver license."

In Lieu Of The Above Minimum Requirements

In lieu of the the education requirement only, be a graduate of a physical therapy educational program approved by the S.C. State Board OR provide satisfactory evidence that education is equivalent to the requirements of physical therapists educated in the U.S. educational programs as determined by the S. C. Board of Physical Therapy Examiners.
