Job Summary

Registered Nurses in the Home Care Services department at UC Davis Health provide care to adult and pediatric patients with a variety of health care needs including post-operative, medical, palliative, and end-of-life care. Patient conditions include both acute and chronic conditions spanning across many care specialty areas and age ranges.

The Home Care Services Department is comprised of the Home Health, Hospice, and Palliative Care programs. These programs offer patients the opportunity to receive skilled nursing care in the comfort of their home. Home care nurses work collaboratively in an inter-professional team environment to provide the highest level of patient-centered care focused on supporting patient goals.

The HHRN II/III identifies and implements nursing interventions and evaluates results for a wide variety of patient populations, directs and acts as resource to other members of the nursing team as assigned, and promotes compliance and support for policies. The primary responsibilities for this position consist of providing total, comprehensive, continuous, and patient-centered care that includes assessing, documenting, planning, implementing, teaching, developing, and evaluating a comprehensive nursing plan of care; delivering direct patient care; ensuring patient satisfaction; training and delegating duties; and adhering to university policies and procedures as well as federal, state, and local regulations.

Autonomous nursing care is the ability of a nurse to assess and provide nursing actions as appropriate for patient care populations based on competence, professional expertise, and knowledge. The nurse is expected to practice autonomously consistent with ANA Standards of Practice and Professional Performance. Independent judgment is expected to be exercised within the context of interdisciplinary and multidisciplinary approaches to patient care.

Provides direct patient care to a diverse patient population. Applies knowledge, experience, and judgment to determine the importance of a situation, set priorities, and use abstract thought to evaluate clinical situations. Participates in patient and staff education. Demonstrates leadership skills. Demonstrates accountability for own practices as defined by the Nurse Practice Act.

Minimum Qualifications

  • Two (2) years of experience as a home health case manager in a Medicare-certified home health agency.
  • Valid California Registered Nurse License without restrictions.
  • Valid California driver's license.
  • American Heart Association (AHA) Basic Life Support (BLS) or UC Davis Health CPR Certification
  • Verification of graduation from an accredited school of nursing.
  • Previous experience in adult patient care with a variety of health care needs including: post-operative, medical, palliative, and end-of-life care, for acute and chronic conditions across many specialty areas.
  • Direct experience and/or education with home health.
  • Ability to read, write, speak, and understand the English language. Ability to utilize medical terminology appropriately. Knowledge of current trends in nursing. Commitment to excellence in patient care. Effective interpersonal skills. Demonstrates accountability and professionalism. Adapts to change in a positive, professional and productive manner. Knowledge of legal implications in clinical practice and documentation. Knowledge of performance improvement and evidence-based practice. Basic computer skills. Ability to assess, plan, implement and evaluate patient care, taking into consideration protective interventions, development (behavioral and physical) and physiological characteristics specific to the patients.

  • CHPN
  • BSN preferred.
  • Two (2) years inpatient experience as a bedside RN on a nursing unit

Our Home Health program is nationally ranked among the highest in patient satisfaction. Home Care Services is made up of three primary programs: Hospice (inpatient and home care), Home Health, and Palliative Care. These programs offer patients the opportunity to received skilled nurse care in the comfort of their home. Home care nurses work collaboratively in an inter-professional team environment to provide the highest level of patient-centered care focused on supporting patient goals. This position would be a part of the Home Health program.

Work Environment

Valid California drives license is required and employees must participate in the DMV pull notice system. Must demonstrate ability in assessment, planning, implementing and evaluating patient care, taking into consideration protective interventions, development (behavioral and physical) and physiological characteristics specific to the patients. Has mandatory reporting responsibilities for domestic violence, child abuse and elder/dependent abuse. Ability to safely lift 25 pounds and assist in moving patients, and to stand on feet over extended periods. Must be able to speak well enough to communicate over the telephone and hear well enough to take vital signs. Must be able to travel to patient homes. Ability to float to other areas in home care. Some restricted vacation due to program needs is possible. Must work evening, on-call, weekends, holidays and overtime as defined by the collective bargaining agreements. Will act as relief night charge nurse on occasion. References reflect an overall meet or exceeds. This position may be subject to a criminal background investigation, drug screen, Live Scan fingerprinting, medical evaluation clearance, and functional capacity assessment. The University of California has implemented a SARS-CoV-2 (COVID-19) Vaccination Program SARS-CoV-2 Vaccination Policy (ucop.edu) covering all employees. To be compliant with the policy, employees must submit proof of vaccination or a University-approved exception or deferral.
