General Purpose of Job: Provides comprehensive pharmaceutical care to Emergency Department patients through integrated drug distribution, clinical services, research and teaching. Responsible for the maintenance, development, and implementation of current and future clinical pharmacy programs and services. Provide clinical pharmacy services, assist in the department's Associate development programs, and provide pharmacy resident and clerkship training and instruction to on-site pharmacy students. Essential Duties: This job description is intended to cover the minimum essential duties assigned on a regular basis. Caregivers may be asked to perform additional duties as assigned by their leader. - Perform clinical pharmacokinetic dosing service in a timely and accurate manner.
- Provide drug information services.
- Regularly interface with individual medical services and improve the quality of pharmacy services to those patients.
- Audit and assure pharmacy residents and quality of existing clinical programs.
- Assist in the clerkship training, instruction, supervision, and evaluation of students assigned to this site.
- Prevent, detect, monitor, document, and report adverse drug reaction and medication errors.
- Participate in the drug therapy management of medical emergencies (code, trauma, stroke, etc.)
- Reconcile pharmacy and nursing medication records as necessary.
- Optimize pharmacotherapy regimens utilizing current evidence-0based recommendations.
- Document clinical consultations, interventions, and cost avoidance for pharmacy within the ED department.
- Monitor the use of expensive medications to assure there is use consistent with approved criteria (e.g.; alteplase, snake antivenom, Factor VII, etc.)
- Provide staff development support to nurses and physicians.
- Dispense medications to inpatients and outpatients in compliance with prescriptions; provide information on medication usage to patients to facilitate the healing process.
- Assess and advise physicians of accepted formulary products, alternative substitutions and/or recommend more efficacious medications for patient therapy.
- Supervise and is responsible for work performed by pharmacy technicians and pharmacy interns on a daily basis.
- Provides accurate, efficient, and friendly service to the nursing units. Supports and assists the other members of the decentralized team. Completes the functions identified in the decentralized job descriptions and assist all associates with all other functions to assure complete, timely and accurate pharmacy services.
- Compounding caregivers adhere to the prescription or medical order for the strength, quality and purity of compounding preparations. Compounded products are packaged, labeled and dispensed according to current laws and regulations.
- Work collaboratively with physicians, nurses, and other staff to facilitate transitions of care.
Job Requirements
General Requirements |
* Valid State of Michigan Pharmacy license in good standing with the State of Michigan required. * Valid State of Michigan Controlled Substance license in good standing with the State of Michigan required. * Completed a Pharmacy Practice Residency; OR BCPS (Board Certified Pharmacotherapy Specialist); OR minimum 2 years of experience working as an ED pharmacist in a level 1 trauma center. |
Work Experience |
* ED pharmacy experience preferred * Minimum of five years experience as a hospital pharmacist preferred |
Education |
* Graduate of an accredited College of Pharmacy. Bachelor's Degree in Pharmacy |
Specialized Knowledge and Skills |
* Demonstrates effective oral, written and phone communication skills * Demonstrates ability to use a keyboard as may be required to perform the essential duties of the job. |
