WHAT YOU WILL BE DOING (ESSENTIAL DUTIES) ...

We are seeking an experienced Case Manager to join our Intensive In-Home Child and Adolescent Psychiatric Services program (IICAPS). The Case Manager will play a crucial role in providing home-based clinical case management to children with psychiatric illnesses and their families.

Essential Duties:

  • Provide home-based case management services to children and their families.
  • Participate in daily team meetings to review client visits, concerns, and coordinate care internally and with external organizations.
  • Co-facilitate family sessions and collaborate on clinical treatment goals with families.
  • Work toward stabilizing and maintaining children in their homes and communities.
  • Develop strength-based comprehensive assessments and treatment plans.
  • Apply model-specific concepts to aid clients in understanding and managing their symptoms.
  • Maintain family and collateral contacts to ensure quality, client-centered services.
  • Assess and involve natural supports in treatment planning and discharge processes.
  • Provide case management, clinical, and crisis intervention supports as indicated, collaborating closely with assigned clinicians during crises.
  • Complete self-administered urinalyses on a weekly basis if required by the model.
  • Demonstrate competency in child development, substance use, and provide trauma-sensitive and culturally competent care.
  • Utilize Electronic Health Records (EHR) to document all services, client and collateral contacts, and appointments.
  • Attend all model-required meetings and agency trainings.

WHAT WE EXPECT OF YOU...

  • Minimum of 2 years of experience in the Human Services field.
  • Bachelor's degree in a related field (i.e psychology, social service, counseling with an expressed interest in continuing in the human service field).
  • Exceptional verbal and written communication skills to engage with clients and stakeholders effectively.
  • Self-motivated and independent, with the ability to prioritize tasks and meet deadlines.
  • Valid Driver's License and reliable transportation to travel to facilities throughout the area.
