Responsibilities

Summary

Under general supervision, the Lead Case Manager position leads the case management staff by providing guidance and training as well as support and structure for participants. The Lead Behavior Change Manager assists in developing program plans, manages a caseload of participant cases, and administers the policies and procedures of the program as directed by the contracting agency and the Program Manager.

Primary Duties and Responsibilities

  • The Lead Case Manager leads the activities of the Behavior Change Manager by assisting them in the work, answering more difficult or complex questions, providing guidance and training to new staff, and reporting to management unusual or complicated case management issues.
  • Manages a caseload of participants using EBP principles; conducts risk/needs assessments and creates individual Behavior Change Plans for each assigned participant; meets regularly with participants and documents participants' progress or regress in company database.
  • The Lead Case Manager prepares written personalized programs for each participant to establish measurable criteria of expected behavior and accomplishments and a time frame for achieving specified goals; reviews plan with participants on a weekly basis and documents progress revisions, so they are gaining the life skills necessary to make a successful reentry into the community.
  • May orient new participants to the program by having them complete paperwork or introducing them to other participants.
  • The Lead Case Manager makes entries into confidential participant files to provide the structure for case analysis and programming planning and to provide documentation of program participation; ensure files are secure to protect participant confidentiality.
  • Participates in staff meetings by sharing observations and recommendations regarding the facility, participants, and staff relations to identify and solve problems and function as a unified team.
  • The Lead Case Manager maintains confidentiality obtained through job duties regarding employees, vendors, participants, and outside agencies so sensitive information is only given on a 'need to know' basis.
  • Reports all program violations to the appropriate agency or official via the established reporting parameters. The Lead Case Manager conducts orientation meetings with new participants to explain program rules and regulations; verbally explains the participant contract in detail to ensure the participant understands his/her responsibilities.
  • Complies with work rules and policies as explained in the company Employee Handbook; responds positively to directives from managers and supervisors in a cooperative spirit to ensure conformance to standards.
  • Adheres to attendance policy to ensure adequate coverage of the program and ensure the safety and security of participants.
  • Complies with safety rules; takes appropriate corrective action to ensure work is performed in a safe manner and without injury to self or others.
  • Communicates effectively with all levels of staff in a manner that promotes a team spirit and sense of cooperation.
  • Performs other duties as assigned.

Qualifications

Minimum Requirements

  • High School Diploma or equivalent. A Bachelor's degree in Human Services discipline or equivalent experience preferred.
  • One (1) year of experience as a Case Manager or three (3) years of work experience in a related field.
    • Work experience may be substituted for academic studies, exchanging one (1) year of work experience in a related field for one (1) year of academic experience.
  • Substance abuse or domestic violence certification or the ability to become certified preferred.
  • Bilingual in English/Spanish preferred, but not mandatory.
  • Ability to interact in a professional manner in both individual and group counseling settings.
  • Ability to collect and analyze data and develop required related reports.
  • Ability to work with a team of professionals and have the ability to implement team strategy approaches to work assignments.
  • Possesses a thorough understanding of the Eight Effective Principles of Intervention.
  • Demonstrated experience leading a group of employees.
  • Excellent organizational skills and attention to detail.
  • Able to operate a PC with a basic knowledge of Microsoft Office Suite, including Word, Excel, and Access.
