The salary of the finalist selected for this role will be set based on a variety of factors, including but not limited to departmental budgets, qualifications, experience, education, licenses, specialty, and training. The above hiring range represents the University's good faith and reasonable estimate of the range of possible compensation at the time of posting.

Position Summary

Columbia University Irving Medical Center, Department of Medicine, is seeking a Nurse Practitioner (NP) to work within the outpatient practice of the Center for Advanced Cardiac Care (CACC) specializing in Advanced Heart Failure, Pulmonary Arterial Hypertension, LVAD, and Cardiac Transplantation programs under the management of the Chief Nurse Practitioner and direction of the CACC Medical Director within the Division of Cardiology. The NP will work collaboratively with Physicians, NP peers, Registered Nurses, other members of the multidisciplinary care team, and administrative support staff within the CACC Program to ensure exceptional patient care.

Responsibilities



  • The incumbent will perform and document comprehensive surgical, medical and psychosocial health histories, physical examinations (document pertinent positive and negative findings), and initiate/maintain a problem/medication list.
  • Order and interpret diagnostic tests, and diagnose/manage chronic as well as acute patient problems.
  • Facilitate and deliver care to a specific subset of patients collaboratively and autonomously.
  • Obtain and review internal and external records/reports.
  • Formulate a goal-directed plan that includes health maintenance, anticipatory guidance and appropriate medical and nursing interventions according to the established protocols.
  • Follow-up/collaborate with the Physician to re-develop the treatment plan as necessary.
  • Counsel/educate patients and their caregivers on disease processes and care management outside of the hospital.
  • Facilitate/coordinate hospital admissions and collaborate with inpatient team for safe discharges.
  • Work with NP peers and the Registered Nurse team to triage daily patient calls and communicate with referring Providers.
  • Work with the multidisciplinary team including but not limited to Social Workers, Nutritionists, Pharmacists, Surgeons, the Palliative Care Team, the CACC Inpatient team, and other specialists as necessary to provide/coordinate care across the continuum.
  • Document all patient/caregiver encounters in the outpatient EMR.
  • Participate, review, disseminate, and incorporate research findings into clinical practice.
  • Participate in weekly multidisciplinary team meetings.
  • Demonstrate responsibility and accountability for Nurse Practitioner practice within respective scope of care and the delivery of safe, competent patient care.
  • Participate in maintaining a safe, therapeutic, and consumer-oriented environment.
  • Portion of time maybe spent in practice at other ColumbiaDoctors sites.
  • Perform other responsibilities as assigned.


Minimum Qualifications



  • Requires a Master's degree in Nursing as a Nurse Practitioner trained in the care of adults, plus one to two years of experience in a hospital setting, preferably in Cardiology.
  • Current New York State Registered Professional Nurse and Nurse Practitioner licenses.
  • Must have NP Board certification.
  • Current NPI certification.
  • Must be eligible for DEA certification.
  • Must be eligible for AHA ACLS & BCLS.
  • Familiarity and proficiency with computer-based programs such as electronic medical records systems, MS Word, MS PowerPoint, and MS Excel.


Preferred Qualifications



  • Adult-Gerontology Acute care Nurse Practitioner (AGACNP) preparation preferred.
  • Experience with the advanced Heart Failure, Pulmonary Arterial Hypertension, and post-Heart Transplant patient populations preferred as well as those patients being considered for/undergoing advanced therapies such as outpatient intravenous and/or inhaled medication support, and/or mechanical circulatory support.


Other Requirements



  • Participation in Medical Surveillance required:

    • Contact with patients and/or human research subjects
    • Wear N-95 respirator
    • Potential bloodborne pathogen exposure
    • Work with known infectious agents or biological toxins where prophylactic vaccination is available


  • Successful completion of applicable compliance and systems training requirements.
