Position Summary

The Care Progression Clinical Supervisor is an expert-level clinician who oversees care progression and care coordination across a defined geography (typically a 'floor' comprised of multiple patient care units). Reporting directly to the Director of Care Progression and Transitions Management, the Clinical Supervisor is a first-line supervisory position and is responsible for supervising a team of unit-based Care Managers and Transition Coordinators. The Clinical Supervisor helps ensure care is delivered in the right place, at the right time, for the appropriate amount of time, with a focused effort to ensure safe/smooth transitions to the next level of care.

The Clinical Supervisor is also responsible for leading daily care briefing and ensuring collaboration amongst the interprofessional team members to optimize care progression. This individual also serves as a coach/mentor/role model on individual units for bedside RNs and other interprofessional staff members. In addition, the Clinical Supervisor identifies barriers to care progression and care coordination, escalates to appropriate parties, and follows through for resolution. The incumbent serves as the clinical subject matter expert and will oversee the assessment, planning, implementing and evaluation for all patient-centered care coordination activities. The Clinical Supervisor is responsible for developing, enforcing, and auditing standard work, and conducting performance evaluations for their assigned team. The individual must also be cross trained to step in as a care manager / transitions coordinator to assist with care coordination standard work, as deemed necessary by the Care Coordination Leadership team.

The success of the Clinical Supervisor is inherent in the incumbent's ability to demonstrate positive, professional communication with patients, families, physicians, departmental, and interdepartmental staff to optimize workflow and to facilitate problem identification and resolution. The employee supports Memorial Care's strategic plan and demonstrates active support and participation in Lean initiatives for continuous performance improvement, patient safety, and patient-centered care. Essential Functions and Responsibilities of the Job

1. Leadership and Direct Supervision: Supervises care coordination team by setting expectations and managing accountability systems. Completes staff performance evaluations and assists with training new staff. Provides guidance to members of assigned team regarding opportunities to improve performance. Serves as a coach/mentor for staff, nurses, and interprofessional team members.

2. Continuum of Care: Clinical subject matter expert for care progression activities, including identifying and escalating barriers. Supervises care coordination and transition planning activities focused on continuity of care and hand-overs between providers of care/services/resources. Serves as a financial steward of hospital resources as demonstrated through utilization and length of stay indicators.

3. Partnership and Collaboration: Partners with the Nursing Operations Leadership to ensure high quality patient care and nursing excellence. Collaborates with Patient Throughput Leadership to ensure seamless and timely patient flow for all admissions, transfers, and discharges.

4. Advocacy and Empowerment: Focuses on advocating/mitigating/negotiating for the right care, at the right time, and in the right place for the right amount of time. Facilitates coordination among interprofessional team, services, and settings involved in the patient's care, with a focus on assuring that the rights and the safety of the patient and family are respected and maintained throughout the continuum of care. Serves as a resource to assist staff with removing barriers to safe patient discharge. Empowers patients to maintain a level of control over their treatment, self-determination, and self-management.

5. Physician Liaison: Supports team members and other interprofessional staff by serving as a liaison with physicians/providers to ensure each patient's plan of care is progressing. Escalates barriers to the appropriate Medical Director / Physician advisor for resolution.

6. Quality Improvement: Embraces a Lean mindset and utilizes the plan-do-study-act (PDSA) performance improvement model. Actively identifies opportunities for improvement, supports data collection, and leads Quality Improvement initiatives.

7. Information Management: Uses software systems, websites, electronic devices, and other information systems per standard work and policy and procedures.

8. Staff Engagement: Fosters employee engagement, inspiring high performing team members and leading by example. Creates a supportive and healthy work environment through clinical and administrative leadership and expertise. Creates an environment that supports and enhances service excellence and the Patient and Family Experience.

*Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications. Minimum Requirements

Qualifications/Work Experience:



  • 5 years' experience as a bedside nurse required.
  • Experience as a charge nurse, team lead and/or experience in care coordination is strongly preferred.
  • Aptitude or desire for supervising/leading a small team of staff members.
  • 7 to 10 years of Case Management experience with no less than 2 years in the acute IP setting
  • Experience with managing cultural diversified patient populations



Education/Licensure/Certification:





  • RN License in the state of California
  • BSN required
  • MSN preferred; will consider a Master's degree in another related health field.
  • Current BLS for Healthcare Providers required.
  • ACM, CCM preferred
