The Home Health Physical Therapist evaluates, plans, and administers comprehensive physical therapy modalities/regimens to patients in their place of residence for the purpose of developing and restoring function through rehabilitative physical therapy as prescribed by a physician. Principal Responsibilities
- Provides initial and ongoing assessment to determine level of function, and any other assessments as required by federal guidelines
- Establishes an in-home treatment plan in coordination with the physician and other care team members.
- Utilizes various evaluation and treatment techniques based on both physical and psychosocial levels of patient function.
- Completes required documentation to complete the patients visits and episodes as required by federal, state and payor guidelines.
- Responsible for planning, evaluation, and treatment implementation of an assigned caseload.
- Provides quality, personalized and evidence-based care and proven interventions.
- Supervises therapy assistants and other home health staff as appropriate.
- Motivates patients during treatment in order to help them function optimally.
- Reduces the need for medications and provides alternatives to surgery.
- Evaluates effects and monitor and communicate progress.
- Consults with physicians regarding change in treatment.
- Writes reports to physicians regarding patient's progress.
- Instructs patients and family/significant others in home programs and activities of daily living.
Education/Experience
- Successful completion of a Physical Therapy program approved by CAPTE, Commission on Accreditation in Physical Therapy Education
- One year of hospital or home care experience is preferred.
- Bachelor's degree or higher in Physical Therapy.
Schedule:
Licenses/Certificates
- Currently licensed by the Illinois State Board of Physical Therapy.
- Current AHA CPR certification is required.
- Reliable means of transportation, valid IL driver's license and vehicle liability insurance required.

- Possible exposure to blood, bodily fluids and infectious diseases
