Job Description
The Licensed Practical Nurse performs assessment & care coordination of patients under the Screening & Prevention program.
Responsibilities
- Process incoming orders, complete chart reviews, triage patient concerns and perform patient phone assessment to determine patient eligibility based on current screening guidelines
- Serve as liason for department by communicating with referring offices, physicians, as well as medical facilities regarding patient concerns, eligibility and clarification of any follow-up required for patient procedures.
- Serve as clinical resource for patients and providers regarding procedure, prep instructions, eligibility qualifications, etc.
- Review prep instructions to patients and submit orders for procedure prep under supervision of physician management.
- Interpret results of all scheduled colonoscopy procedures and ensure results are relayed to patients and ordering providers, as well as documented in chart.
- Administers treatments as ordered by the providers. Responds to patient prescription requests and phone calls in a timely manner.
- Management of patient recall for repeat colonoscopies and directives provided by performing physician.
- Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing electronic medical records on a state-of-the-art touch pad tablet.
- Appropriately labels and provides insurance info for any referral to lab, x-ray, or specialist. Refers according to insurance guidelines and protocol, records and reviews all services and supplies rendered on the superbill for accuracy and proper code selection. Completes work efficiently and accurately at the pace indicated by the flow of customers. During decreases in customers, utilizes time productively for the benefit of the clinics.
- Assists in the provision of patient teaching and discharge planning.
- Maintains current knowledge/certification/licensure.

Qualifications
Minimum Education
- Technical Diploma/Certificate Practical Nursing Required
Licenses and Certifications
- Licensed Practical Nurse Florida or eligible compact state license Upon Hire Required and
- BLS for Healthcare Providers (BLS) American Heart Association Upon Hire Required
Required Skills, Knowledge and Abilities
- Excellent communication skills verbally and written (High proficiency)
- Knowledge of patient care charts and patient histories. (High proficiency)
- Knowledge of patient evaluation and triage procedures (High proficiency)
- Knowledge of appropriate procedures and standards for the administration of medications and patient care aids. (High proficiency)
