Position Summary

The Centering Coordinator provides administrative support to and coordination of the Centering Program at Mary's Center. Overall responsibilities include coordinating activities necessary to achieve and maintain the quality and sustainability of the programs within the clinical practice setting, collaborating closely with the Medical and Nursing team, Social Services team, facilitators and other staff involved to promote group care, leading the Steering Committee meetings, and maintaining organizational and reporting duties. This role requires strong organizational skills, an understanding of group dynamics, and a commitment to improving health outcomes through innovative care models.

Reportability

This position will report directly to the Director of Care Coordination.

Essential Duties and Responsibilities

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.

Program Administration



  • Develop and manage strategies to increase participation in Centering programs
  • Oversee spending, inventory and distribution of supplies, equipment and materials for Centering programs.
  • Conduct ongoing data collection and reporting to evaluate program performance including provider productivity, patient referrals and health outcomes.
  • Assist with research, writing and editing key documents (e.g., proposals, presentations and reports).
  • Set schedules, develop agendas and facilitate Steering Committee meetings.
  • Perform ongoing monitoring, maintenance and problem solving as needed for Centering sessions (reserving rooms, creating calendars, developing agendas for sessions, assisting with documentation, etc.)
  • Perform various administrative duties as necessary to enhance the efficiency and quality of Mary's Center group care program (e.g., scanning and faxing documents, recording minutes, etc).
  • Coordinate with care teams to recruit and enroll appropriate candidates to program for Centering groups.
  • Confirm attendance for group appointments and follow up with patients who do not attend.
  • Provide resources and support services for patients experiencing barriers to care, including linkage with BHAP and Social Services.


Group Facilitation & Training



  • Arrange for new facilitators to receive Centering Facilitation Training.
  • Train Mary's Center staff on group care protocols.
  • Work with volunteers, students and/or interns assisting with group care program.
  • Observe and co-facilitate Centering sessions as needed: assist with patient education in preventative health, self-assessment activities, answer questions, and provide individual counseling and/or referrals.
  • Coordinate community guest speakers to present relevant resources to groups.
  • Ensure staff is meeting expectations and provide appropriate feedback on areas of improvement.


Communications



  • Interact with and be accessible to different stakeholders, ensuring information sharing and communication between groups.
  • Increase patient, staff and stakeholder awareness of and interest in group care program through outreach and marketing strategies.
  • Serve as liaison with Centering Healthcare Institute.
  • Develop and maintain collaborative relationships with other group care practices and networks.


Qualifications

Knowledge, Skills & Abilities -



  • At least one (1) year of relevant experience; two (2) or more years' work experience preferred.
  • High school diploma required. Bachelor's Degree preferred.
  • Thorough understanding of patient/client confidentiality issues and HIPAA compliance/regulations.
  • Experience working with culturally diverse and underserved populations. Awareness and sensitivity of sociocultural issues related to immigration. Demonstrated interest in perinatal health, public health, health education, or related field.
  • Strong project management capabilities required.
  • Ability to multi-task, prioritize and adjust activities appropriately. Excellent organizational skills and attention to detail. Proactive and able to work independently and solve problems as needed. Team player with ability to work collaboratively with staff that works remotely.
  • Advanced computer skills including experience with Microsoft Office (Word, Excel, PowerPoint, etc) and familiarity with creating and/or using charts, spreadsheets and databases a plus.


Language Requirement - Demonstrated ability to communicate effectively with English speakers and speakers of other languages is required. Spanish language proficiency or fluency strongly preferred.

Physical Demands - Regularly required to sit; frequently required to reach with hands and arms, walk, stoop, kneel, crouch, talk or hear; must be able to lift objects up to twenty-five (25) pounds.

Work Environment - Mostly in a typical office or clinic setting with quiet to moderate noise level. May be exposed to extreme cold, heat, and humidity due to outside weather conditions. May be exposed to potentially hazardous blood-borne/respiratory pathogens and bodily fluids which require the practice of Universal Precaution procedures. Opportunity to work a hybrid schedule - remotely & onsite, as needed. Must be available to travel to all sites as needed.
