The Court Collaborative Program provides mental health and substance use disorder services to adult participants in eight Alameda county treatment courts. This team supports participants with assessment and case management services. We link our participants with appropriate SUD and mental health services; support them with their treatment goals and maintain communication with the court team as related to their progress on these goals. The Court Collaborative Team leads collaborative court hearings and provides consultation and training as related to supporting our participants' treatment needs. We work in close collaboration with our many court partners including judges, attorneys, probation and parole officers, and representatives from the Veterans Administration and the Department of Children and Family Services. Our primary goal is to ensure that our participants have access to treatment in order to graduate from treatment court and successfully exit the criminal justice and/or DCFS systems.

Full Time; DAYS 8:30 am - 5:00 pm; Monday - Friday

Expected starting wage range is $39.59 - $48.90. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.

POSITION SUMMARY
The Social Work Clinician III - Court Collaborative Services independently provides clinical and casework services to members served and natural supports. This involves person centered recovery planning and collaborating with other services and agencies. May act in the role of a clinical supervisor of unlicensed staff. This position is for a high level mental health professional to provide all manner of clinical support to the Office of Collaborative Court Services (OCCS).

ESSENTIAL FUNCTIONS
* Demonstrates the Telecare mission, purpose, values, and beliefs in everyday language and contact with internal and external stakeholders
* Conducts psychosocial assessments, treatment planning, group and individual clinical service, case coordination, and documentation
* Independently completes clinical assessments
* Partners with the Court Case Managers to ensure that all members served receive a mental health assessment; may involve coordinating the assessment with the OCCS part-time mental health assessment staff and accessing relevant assessment data from other county agencies including Criminal Justice Mental Health Services (CHMH)
* Accesses mental health records for members served, including hospitalizations, medication histories, past and current diagnosis, and past/current relationships with other mental health case management agencies
* Knows and learns all mental health services available in Alameda County; visits all sites and learns eligibility requirements and referral procedures; works with the Court Case Managers to identify the mental health needs of the members served; understands how members served can access a variety of mental health services including emergency hospitalization, crisis intervention, psychiatric and medication evaluation, individual counseling, family counseling, and ongoing mental health case management
* Attends all pre-court "staffing" meetings and offers a mental health perspective as cases are being discussed; advises and educates the team (including Judge, Parole, Probation. Attorneys, Social Workers and Case Managers) on mental health issues and possible solutions
* Provides consultation and support to Court Case Managers with respect to the mental health needs of members served; provides close support when members served are experiencing mental health distress; in special instances, may provide brief crisis counseling
* Compiles necessary statistical data; compiles periodic reports of activities for evaluation purposes; helps with grant writing; attends conferences and steering committees; creates all administrative systems for delivery of mental health services in the OCCS
* Establishes and maintains a therapeutic relationship with members served
* Assures provision of needed clinical services to assigned members served
* Coordinates ongoing treatment within the program and with outside agencies
* Provides individual and group therapy
* Documents all treatment provided in a thorough and consistent manner
* Collaborates with other staff in the discharge planning process and makes linkages to community services for members served
* Actively participates in multidisciplinary team meetings and recovery planning meetings
* Demonstrates knowledge of de-escalation techniques and crisis communication and management
* Serves as a clinical consultant to recovery team members
* Must evade members served in the event of assaultive behavior and work with on-site Sheriff deputies. Must pass assault crisis/ crisis prevention training
Duties and responsibilities may be added, deleted, and/or changed at the discretion of management.

Position requires transporting Members in own personal vehicle (acceptable driving record and vehicle inspection for safety)

QUALIFICATIONS
Required:
* Master's Degree in Social Work, psychology, or equivalent from an accredited college or university
* Two (2) years of direct service experience in a behavioral health setting
* Licensure (LMFT, LCSW, Licensed Psychologist, LMFT, LPCC, QMHP, QMHA)
* Must be at least 18 years of age
* Must be CPR, Crisis Prevention Institute (CPI), and First Aid certified on date of employment or within 60 days of employment and maintain current certification throughout employment
* All opportunities at Telecare are contingent upon successful completion and receipt of acceptable results of the applicable post-offer physical examination, 2-step PPD test for tuberculosis, acceptable criminal background clearances, excluded party sanctions, and degree or license verification. Position requires driving, valid driver license, a motor vehicle clearance, and proof of auto insurance is required at time of employment and must be maintained throughout employment. Additional regulatory, contractual, or local requirements may apply.

Preferred:
* Previous experience with population to be served and/or work in a similar program setting
* Two (2) years of post-licensure experience in order to provide clinical supervision to license eligible staff

SKILLS
* Ability to complete social histories, risk assessments, mental health status exams, and plans of care
* Ability to work collaboratively with physicians, nurses, and rehabilitation therapists
* Ability to form a therapeutic alliance with members served and families
* Sufficient medical knowledge to assess the needs of members served
* Skills in conducting group therapy
* Ability to advocate for the members served in other health delivery systems

PHYSICAL DEMANDS
The physical demands here are representative of those that must be met by an employee to successfully perform the essential functions of this job.

The employee is occasionally required to sit, bend, squat, kneel, push, pull, and lift and carry items weighing 25 pounds or less as well as to frequently stand, walk, reach, twist and do simple and power grasping. The position requires manual repetitions, deviations, and dexterity and to occasionally drive.
