Clinical Pharmacist I

Major Responsibilities:

  • Develops competency in clinical practice including appropriate drug therapy for patients and prevention/resolution of drug related problems.
  • Develops competency to evaluate patients for drug-drug, drug-food, drug-lab, and drug-disease interactions. Individualizes drug therapy regimens based on patient's size, organ functions, and disease states.
  • Facilitates medication-related continuity of care at admission, transfer and discharge.
  • Reviews and accurately interprets medication orders.
  • Develops competency in dispensing and compounding medications consistent with good pharmaceutical practice including compounding technique, packaging, and labeling.
  • Practices in compliance with department policies & procedures, as well as local, state and federal regulations.
  • Ensures formulary adherence, recommends cost effective therapies to providers, and minimizes waste in daily practice.
  • Actively supports procedures and initiatives to prevent medication events.
  • Reports events through appropriate channels.
  • Actively supports department medication safety and quality improvement activities.
  • Orients/trains new pharmacy team members. May work with interns, residents and students to guide daily activities. May be preceptor-in-training.
  • Provides direction to pharmacy technicians to ensure smooth workflow and service standards.

Licensure, Registration, and/or Certification Required:

  • Pharmacist license issued by the state in which the team member practices.

Education Required:

  • Bachelor's Degree in Pharmacy.

Experience Required:

  • Hospital pharmacy experience preferred.

Knowledge, Skills & Abilities Required:

  • Maintains skills in areas of practice.
  • Strong computer skills.
  • Excellent communication and organization skills.
  • Excellent presentation, facilitation and training development skills.
  • Demonstrated decision making, team building, customer service, and conflict resolution skills.
  • Life support training courses may be required dependent on department discretion

Physical Requirements and Working Conditions:

  • Must be able to stand for long periods of time.
  • Will occasionally lift up to 35 lbs. without assistance.
  • Must have functional speech, vision, and hearing.
  • Will be exposed to chemical hazards.
  • Operates all equipment necessary to perform the job.
  • Exposed to normal office environment.
