Role: Clinical Case Manager Reports to: Program Manager Department: Permanent Housing Location: CSM Fire House:1444 7th St. Santa Monica, CA 90401 Setting: Outreach/ Field Based/Office Schedule: Monday - Friday, 8:00 am - 4:30 pm Status: Full-Time, non-management, non-exempt Benefits: Medical, Dental, Vision, Retirement Planning (403b), Employee Assistance Program (EAP), etc. Summary The People Concern's Santa Monica Homeless Multidisciplinary Street Team (HMST) is seeking a master's level clinical case manager on this multidisciplinary street outreach team. This team is part of a City of Santa Monica strategy to combat homelessness. The goal of HMST is to target the highest utilizers of the City of Santa Monica's emergency resources (police, fire, and hospitals). The skilled interdisciplinary team uses best practices for engaging chronically homeless individuals with co-occurring mental health, substance use, and/or medical issues. A low client-to-staff ratio is used to maximize effectiveness. The team works closely with each client to develop individualized service plans. The team helps enroll clients in public benefits (i.e. General Relief, SSI/SSDI, Medi-Cal), submit housing applications, and access appropriate medical and behavioral health treatment. Essential Duties and Responsibilities: 1. Provide field-based services, meeting currently homeless clients where they are in order to eliminate barriers to services. 2. Assess the mental health service needs of clients. 3. Engage clients in developing mutually agreed-upon goals documented in each client's confidential service plan. 4. Provide risk assessment and clinical evaluations for clients who are potentially gravely disabled and in need of hospitalization and referral to higher levels of care. 5. Link clients with appropriate medical, mental health, substance abuse, and vocational resources. 6. Assist clients in obtaining all benefits to which they are entitled, including applying for and advocating for SSI benefits. 7. Provide opportunities for clients to build a supportive peer community. 8. Build relationships with County mental health and other resources to increase collaboration and responsiveness to clients with severe mental illness and addiction. 9. Complete all charting in a timely manner. 10. Collaborate and communicate effectively with police officers and local hospital staff to reduce police contact and ED visits by targeted populations. 11. Work effectively on an interdisciplinary team and collaborate with the Program Manager to ensure a high level of clinical oversight. 12. Other duties as assigned. Qualifications: 1. Master's degree or higher in a mental health field (MSW, MFT, PhD, PsyD) 2. Demonstrated knowledge of clinical case management. 3. Experience in dealing with issues faced by population served (serious mental illness, substance abuse, and physical health problems). 4. Excellent writing and charting skills. 5. Knowledge of community resources. 6. Able to maintain an effective working relationship with Santa Monica community partners (including police, firefighters, city staff, etc) and maintain and build professional rapport 7. Ability to self-motivate, work independently, and communicate effectively. 8. Able to work collaboratively as part of an interdisciplinary team 9. Valid CA Driver's license, reliable car, auto insurance, and an acceptable driving record EXPECTED BEHAVIORS OF ALL STAFF
