Responsibilities

* Oversees Healthy Start CHWs and provides oversight in documentation, patient care, and clinical competencies, and is a clinical expert for policies, procedures, documentation, and skills.
* Provides ongoing professional development opportunities to CHWs in the areas of health literacy, maternal-child health, substance use disorder, safe sleep, and other topic areas as needed.
* Assists in the onboarding, training, and development of new CHWs, including performance appraisals.
* Assists CHWs and community partners in the decision-making process for improving linkage of patients and families with community resources.
* Provides direct counselling services to Healthy Start Families and navigates high-need situations.
* Remains informed of trends and developments in available community resources.
* Participates in quality improvement and evaluation activities and operates in accordance with NCH social work program standards and expectations.
* Performs administrative tasks related to the Healthy Start grant.
* Maintains safety and quality as priorities, providing auditing and education for safety and quality initiative.



Qualifications

Education Requirement:

    Master's Degree in Social Work or Education, required.

Licensure Requirement:

  • Licensed Social Worker or equivalent license, required.

Skills:

  • Excellent communication and interpersonal skills for interaction with patients, families, and community providers.
  • Ability to work as a part of a team, with diverse populations and healthcare providers in clinical settings.
  • Strong attention to detail and organizational abilities.
  • Sound decision making skills, with the ability to maintain highly confidential information.
  • Proficiency with computers and MS Office.
  • Ability to prioritize tasks, problem solve, and work autonomously.

Experience:

  • Experience using trauma-informed and person-centered frameworks to support families, required.
  • Resident of Southeast Ohio with understanding of Appalachian health needs, required.
