Overview

The Admissions LVN provides professional nursing care under supervision of RN for assigned patients across the life span (perinatal, pediatrics, and adults) according to established standards and practice. Participates as a team member of the admissions team to ensure appropriate resources to all internal/external customers. Follows The Elizabeth Hospice rules, policies, procedures, applicable laws and standards addressed in the Compliance Program. Responsible for carrying out the mission, vision, values and quality commitment of The Elizabeth Hospice.

The Elizabeth Hospice is San Diego's oldest non-profit hospice of 45 years. We are growing and looking for talented individuals to join our high performing team and support our mission. We serve all of San Diego county and south Riverside county with multiple hospice office locations.

Our Mission: To enhance the quality of life for those nearing the end of life's journey and for those who grieve.

Salary: $30 - $38 per hour. Compensation is determined by years of licensure.

Work Schedule: Full-Time, hours and days vary, 8-hour shifts beginning between 10am and 1pm. Candidate must be available to work 2 weekends per month.

Work Location: San Diego County, CA

Travel: Travel is required for patient visits.



Responsibilities

    Assists RN in analyzing situation, anticipating problem areas and taking appropriate course of action.
  1. Evaluates the response of patient, relative to the expected outcomes in a safe and timely manner.
  2. Performs licensed nurse skills as stated in basic skills checklist, policies and procedures, standards of care, critical competencies based on scope of practice.
  3. Under the supervision of the RN, implements and documents teaching plan based on: mutually assessed needs of patient and family, in a timely manner, teaching strategy suitable for the learning ability and cultural background of the patient and family.
  4. Adheres to direction provided by RN on the plan of care for the patient.
  5. Responsible for delivering services under the supervision of and in collaboration with RN
  6. Documents completely all care provided to patient including telephone interaction with other staff members. Charting standards are followed.
  7. Enters or retrieves data from established computer files.
  8. Orients, instructs and trains assigned personnel.
  9. Performs Phlebotomy as needed.
  10. Provides support to agency in completion of assigned audit and other office duties as assigned.
  11. Will be cross-trained in Clinical to assist the hospital liaisons with discharge of patients from the hospital to home, Triage, and Access Center and as a Program Representative.
  12. As a resource LVN, assists RNs in ordering DMEs' and medication as needed and assists with placement of patients in SNF, ALF, or RCFE
  13. Under the direction of the RN manager of the access center, may assists RN in admission of patients to SNF.
  14. Adhering to the E-way standards is a requirement in performing all job duties.


Qualifications

  1. Vocational/Specialized Training
  2. Minimum 12 months experience as an LVN.
  3. Preferred experience of 1-2 years in acute care setting, home health or hospice.
  4. Current California LVN license.
  5. Current CPR certification and negative TB skin test.
  6. Ability to answer phones, maintain patient charts, and schedule appointments.
  7. Phlebotomy certification preferred.
  8. CHPLN certification preferred.
  9. Spanish speaking may be preferred or required.
  10. Current California driver's license and proof of auto insurance required.
