Overview

Physician Affiliate Group of New York (PAGNY) is adding a Patient Safety Officer to the group at NYC Health + Hospitals/ South Brooklyn Health. The Hospital is part of NYC Health + Hospitals. South Brooklyn Health is a redesigned health care campus featuring the new 11-floor Ruth Bader Ginsburg Hospital which provides emergency care, modern labor & delivery suites, and state-of-the-art surgical services. The new Health & Wellness Institute offers outpatient services-from primary and preventive care to specialty services for children and adults-all designed to keep our community healthy and accommodates more than 250,000 outpatient visits, more than 89,000 emergency room visits, and with 371 beds, more than 15,000 inpatient admissions annually.

The hospital maintains a strong academic affiliation with SUNY Downstate Health Sciences University and NYIT College of Osteopathic Medicine to maintain its high standards of healthcare delivery.

Responsibilities

The Patient Safety Officer will join the administration team at South Brooklyn Health.

Responsibilities Include:

    Safety Event Reporting: Track, trend and manage event reporting for "near misses" or "good catches"; implement and maintain a "good catch" celebration program.
  • Root Cause Analysis: Participate in or lead root cause analysis discussions; participate in performance improvement for corrective action plans
  • Leapfrog: Coordinate data collection and entry for Leap Frog twice a year along with Quality department; identify and participate in performance improvement projects related to low scoring areas of Leapfrog
  • Culture of Safety Survey: Organize distribution of the survey every two years; categorize the results by department and discipline to share with executive leadership, department leadership and front-line staff; track action plans
  • National Patient Safety Goals: Track and trend data/improvements for National Patient Safety goals on quarterly basis at a minimum; regularly share updates in trends to executive leadership; utilize recognized/standardized tools for analysis such as run charts etc. and approved methods of performance improvement i.e. PDSA.
  • Patient Safety Meetings: Attend monthly System-Wide Patient Safety Council meetings; coordinate regular (monthly/quarterly) PS meetings at facility; review reported incidents, trends of national patient safety goals, good catches, and any other pertinent projects
  • Patient Safety Rounding: Participate in and coordinate Executive Leadership Rounds on a regular basis (weekly-monthly); collect, track, and coordinate action plans for shared patient safety concerns; share PS improvements regularly with front-line staff
  • Patient Safety Education: Complete TEAMSTEPPS training for new employee orientation or for requesting units; regularly educate staff on National Patient Safety Goals, how to report safety events and the importance of reporting
  • Patient Safety Projects: Help mentor and sponsor patient safety projects and encourage submission to conferences such as IHI or for awards such as HANYS Pinnacle Award



Qualifications

  • Certification in Safety or Quality (e.g. CPPS) preferred; if not achieved prior to hire will be completed within 2 years of start date
  • Proficiency in EPIC, specifically with data mining and reporting
  • Proficiency in Microsoft Office programs
  • Valid New York State license and current registration to practice as a Registered Professional Nurse issued by the New York State Education Department (NYSED) and two (2) years of clinical and/or administrative experience in coordinating and monitoring initiatives, activities, and programs related to patient safety, quality improvement, and related area(s), analyzing, managing, and reporting patient safety data and performing other functions related to patient safety in a hospital or health care setting;

OR

  • Doctoral degree from an accredited university in Medicine, Nursing, Public Health, Public or Hospital Administration and one (1) year of experience.

OR

  • Master's degree from an accredited college or university in Public Health, Public or Hospital Administration and three (3) years of experience

OR

  • Bachelor's degree from an accredited college or university in Public Health, Public or Hospital Administration and five (5) years of experience;

OR

  • A satisfactory equivalent combination of training, education and/or experience.
