Join our dedicated Outpatient Heart Failure team at ProHealth's Heart & Vascular Center, where you'll play an essential role in supporting patients with a range of cardiovascular conditions. This position is perfect for someone passionate about cardiac care and looking to make a meaningful impact in patients' lives. 7:30-4:00 with variability by days; Heart & Vascular Center on campus of Waukesha Memorial Hospital as primary location.; Oconomowoc clinic as secondary location - one day each week for clinic support. As a Clinical Support Associate, you'll assist in outpatient care, helping patients manage chronic conditions, providing education, and supporting diagnostic and therapeutic services. Provides direct care to patients under the supervision and delegation of the Nurse Practitioner, Physician Assistant or Physician within ProHealth Medical Group. Provides care in partnership with the patient and their family, with the goal of supporting an optimal healing community.
