The Hundley Center is a division of VCU Health Community Memorial Hospital. Our 140 beds are certified for Medicare and Medicaid. Individuals may receive extended care (short or long term) in one of our two units. We provide a variety of services to include speech/occupational/physical therapy, respiratory services, dementia care, IV therapy, end of life care, and wound care. The Nursing Supervisor LTC is responsible for unit leadership/management and acts as a clinical resource. The Nursing Supervisor LTC ensures excellent patient care is provided by nursing staff, including RNs, LPNs, CNAs and Nursing Assistants, to meet the needs of the residents. The Nursing Supervisor LTC is responsible for ensuring care is delivered in a healthy, healing and respectful environment to all residents, visitors and staff.

Essential Job Statements

  • Provides leadership and management on the unit, to include conducting staff meetings, performance counseling, making assignments, assessment of patients, conferences with family or responsible parties, etc.

  • Provides direct supervision to clinical staff (including RNs, LPNs and CNAs). Conducts performance evaluations annually and as otherwise needed. Manages the disciplinary process for performance concerns. Determines approval for requested time off.

  • Monitors throughput of residents' daily activities, appointments, therapy, medical care, etc.

  • Acts as a clinical resource, promoting resident-centered care and monitoring and teaching resident care within the LTC facility.

  • Evaluates staffing patterns to determine and maintain adequate staffing levels within the long term care facility.

  • Identifies and resolves potential safety and environment problems within the LTC facility.

  • Completes multiple QAPI audits each month, writes action plans to address noted areas for improvement, and implements action plans with staff. Monitors LTC quality measures and leads and/or participates in corresponding quality and performance improvement activities within the LTC facility.

  • Maintains understanding of all LTC regulations and follows changes in these regulations.

  • Monitors and maintains medical as well as other material supply levels within the LTC facility.

  • Supports survey readiness within the LTC team by auditing and monitoring work of the staff.

  • Effectively leads and facilitates meetings.

Patient Population

Demonstrates the knowledge and skills necessary to provide equitable care appropriate to the age of the patients served on their assigned unit.

Demonstrates knowledge and related competencies of the principles of growth and development of the life span and possesses the ability to assess data reflective of the patient's status and interprets the appropriate information needed to identify each patient's requirements relative to their age, specific needs and to provide the equitable care needed as described in departmental policies and procedures.

Employment Qualifications

Required Education:

Diploma or Associate's Degree from a professional nursing program recognized and/or approved by the corresponding state's Board of Nursing.

All registered nurses without a Bachelor's degree in Nursing (or higher) will be required to enroll in an approved RN to BSN program within two years of their start date and to complete the program within five years of their start date.

Preferred Education:

Master's Degree in Nursing or a related field.

Licensure/Certification Required:

Current RN licensure in Virginia or eligible compact state

Licensure/Certification Preferred:

Certifications and/or registrations in functional area

Diversity Equity and Inclusion certification preferred

Minimum Qualifications

Years and Type of Required Experience:

Minimum of five (5) years of clinical experience with at least two (2) as an RN. Clinical experience can include CMA, LPN, or other clinical technical roles in healthcare.

Two (2) years of clinical supervisor or other clinical leadership experience.
