General Summary of Position

The Home Health Aide will provide personal, in-home care for the agency's patients.

Primary Duties:

  • Attends Agency regional office and team meetings as required. Attends in-service educational programs. Demonstrates flexibility in adjusting to changes in changes with procedures and in daily schedule. Maintains effective working relationships with other departments and participates in multidisciplinary quality and service improvement teams. Participates in meetings and on committees and represents the department and agency in community outreach efforts. Enhances growth and development and enriches personal knowledge and skill through participation in educational programs and affiliations.

  • Cleans direct patient care area, as necessary. Identifies safety hazards in the home and reports it to the team's Operations Director or primary case-manager. Reinforces with patient/family ways to remove identified safety hazards in the home. Performs personal care activities (bathing, dressing, oral hygiene etc.). Performs other related activities (light meal preparation, personal laundry etc.). Assists with transfers and range of motion exercise programs as instructed by professional staff. Performs procedures as delineated on HHA (Home Health Agency) Skills Competency Checklist as instructed by professional staff.

  • Document services in accordance with Agency medical records and timekeeping policies, including the absence of patients from their homes. Documents patient progress reports clearly and concisely for multidisciplinary case conference.

  • Recognizes signs of emotional distress (such as anger and depression). Encourages patient mobility and socialization during visits. Recognizes and responds to special needs of patients/families experiencing terminal illness.

  • Reports identified alterations in patient behavior or mental status to primary case manager or the team's Operations Director. Reports identified patient care or family health or social problems to the team's Operations Director or primary case manager. Reports all problems in carrying out assigned activities to primary case manager or the team's Operations Director. Collaborates with case manager or professional staff in the revision and implementation of the Home Health Aide plan of care. Informs the team's Operations Director of the need to change the patient's visit schedule.

Qualifications:

  • Ability to read, write, and perform arithmetic calculations.

  • CNA (Certified Nursing Assistant) / HHA (Home Health Aide) in the District of Columbia.

This position has a hiring range of $19.17 - $31.36
