Schedule

  • 8-hour day shifts, Friday-Sunday (24 hours/week)

  • Enjoy a shift differential of +$3.10/hour.

Position Summary

As a Medical Social Worker II, you'll play a vital role in our interdisciplinary care team. You'll provide comprehensive psychosocial support to patients and their families in a home care and/or hospice setting. Your expertise will help address emotional, social, and practical challenges, ensuring a seamless transition through their care journey.

Key Responsibilities

  • Conduct comprehensive psychosocial assessments to identify patient needs and barriers to care.

  • Develop and implement individualized care plans in collaboration with the interdisciplinary team.

  • Provide crisis intervention, counseling, and support to patients and families.

  • Coordinate with community resources to ensure optimal care and support.

  • Maintain accurate and detailed documentation.

  • Facilitate and participate in care conferences and family meetings.

Qualifications:

  • Master's degree in Social Work required.

  • Vermont LMSW or LICSW licensure preferred.

  • Minimum two years of relevant experience in healthcare.

  • Strong communication and interpersonal skills.

  • Ability to work independently and as part of a team.

  • Passion for providing compassionate care.
