Location:Information Systems - Orion Building - 6630 Orion Dr, Fort Myers, FL 33912 Department: LPG Pharmacy Services Work Type: Full Time Shift: Shift 1/8:00:00 AM to 4:30:00 PM Minimum to Midpoint Pay Rate:$51.54 - $67.01 / hour SummaryThe LPG Pharmacist will be integrated into primary care with a focus on improving patient outcomes through assuring optimal prescribing practices consistent with evidence-based literature and treatment guidelines. Conduct medication use evaluations, and support patient access and adherence to therapy. Utilize and leverage analytic data to determine which patients will benefit from interventions, medication education and collaborate with primary care providers and specialists in the care of these patients. Perform detailed medication reconciliation. Conduct virtual or face to face visits to provide comprehensive medication management. Provide recommendations to providers regarding utilization of high-cost medications. Participate in the development of quality measures and quality improvement initiatives. Provide educational presentations to providers. Provide technician supervision to ensure accurate and timely submissions of prior authorizations. Other requirements Preferred: Strong communication and interpersonal skills, analytical skills, and leadership skills. Collaborates well in a team environment. Uses effective conflict resolution skills. Proficient in use of the EMR. Willing to serve as a preceptor to pharmacy students and residents. Educational requirements Preferred: Doctor of Pharmacy degree OR BS Pharmacy degree with equivalent and acceptable experience from a College of Pharmacy Accredited by the American Association of Colleges of Pharmacy with at least 5 years of experience. Certification requirements Preferred: Collaborative practice agreement certificate Ambulatory Care Pharmacy Specialty Certification (BCACP) Minimum years of experience Preferred: 3 years of inpatient or ambulatory pharmacy experience, or completion of PGY 1
BS Pharmacy degree with equivalent and acceptable experience from a College of Pharmacy Accredited by the American Association of Colleges of Pharmacy. Experience Requirements
Five years of professionally responsible pharmacist experience required for Bachelor's Degree holders. One year recent ambulatory experience as pharmacist or pharmacy intern for PharmD candidates. State of Florida Licensure Requirements
