Works with patients who present with AODA and/or Mental Health concerns. Completes psychosocial assessments, treatment plans, and ongoing treatment with individuals, groups, and families in cooperation with patient and multidisciplinary team. Consults and coordinates care of patient with physician, providers, and community agencies. Delivers behavioral health care in accordance with patient evaluation, treatment plan, and within the scope of their license. Maintains pertinent patient data in medical record in accordance with state, Joint Commission, and Mercy Health System guidelines. Performs other duties as assigned. Complies with all Mercy Health System, State, and Joint Commission policies and regulations. Must be willing to travel between treatment sites, on occasion, when asked to do so during situations where program coverage is required, meetings, and/or system in-services are required.
Responsibilities
Essential Duties and Responsibilities
- Interviews clients, reviews records, and confers with other professionals to evaluate condition of client.
- Formulates program for prevention, treatment and rehabilitation of client.
- Counsels clients individually and/or in group sessions to assist client with their mental health issues and/or in overcoming alcohol and drug dependence through monitoring the client (s) treatment success and adapting treatment as needed.
- Counsels family members to assist family in dealing with and providing support for client.
- Refers client to other support services, as needed, such as medical evaluation and treatment, social services, and employment services.
- Prepares/maintains current reports/records.

Education and Experience
Master's Degree in Counseling, Social Work, Psychology or other related field.
Certification/Licensure
Licensed to independently provide both mental health and substance abuse treatment in Wisconsin. Mental health license includes LCSW, LPC, LMFT. Substance abuse license includes Clinical Substance Abuse Counselor (CSAC); Substance Abuse Counselor (SAC); or LCSW, LPC, LMFT with attached Substance Use Disorder specialty. #MHT123
