SUMMARY

This current opening is part time, benefit earning working 0.6FTE (48 hours/pay period) Day shift during the weekdays.

Within the context of Hennepin County Medical Center Relationship-Based Care practice model and using skills in wound, ostomy and continence care and prevention, the Wound, Ostomy, and Continence (WOC) nurse consults with direct caregivers and nursing leadership to treat and develop a comprehensive action plan to reduce and eliminate pressure ulcers, care for ostomies and fistulas, and manage continence issues. This includes participating in data gathering, database management and reporting of findings.

RESPONSIBILITIES




  • Designs, and evaluates population-based programs of care, with an emphasis on wound and ostomy care
  • Assists with coordination and facilitation of the Skin Team
  • Works with, supports, and acts as expert resource in patient care issues related to wound, ostomy and continence
  • Develops, monitors, and evaluates protocols for wound care treatment and strategies
  • Provides wound and ostomy evaluation and treatment in the inpatient and outpatient setting. Provides patient and family education and follow-up
  • Partners with nursing staff, physicians and support personnel to provide interdisciplinary wound care management. Provides consultation and recommendations for products and supplies to support optimal wound care management
  • Develops an ongoing education program for healthcare team members appropriate for the novice, competent, expert practitioner and teaches classes related to skin safety
  • Provides education to clinical and medical staff on prevention strategies and treatments
  • Partners with Purchasing to make recommendations for products and supplies and facilitates implementation of these products, using best practice and financial data Develops and maintains clinical algorithm for beds/mattresses, and wound care products
  • Promotes Evidence Based Practice and Research and participates as appropriate
  • Partners with and reports findings to Performance Improvement and Quality regarding skin safety initiatives and issues
  • Performs other duties as assigned

QUALIFICATIONS

Minimum Education/ Work Experience:



  • Bachelor's degree in Nursing required


Preferred Qualifications:



  • Experience developing adult education programs and leading interdisciplinary teams.
  • Recent Wound and Ostomy nursing experience
  • 2 years of recent, acute care hospital experience;
  • Involved in a professional nursing organization

License/Certifications:



  • Valid license as a Registered Nurse issued by the State of Minnesota
  • Certification in Wound and Ostomy care (CWON), or in process to obtain within 6 months.
