Acentra seeks a Children's Service Support Facilitator to join our growing team!

Job Summary

The Children's Service Support Facilitator will conduct comprehensive level of care assessments for children identified by the State Department of Child Protective Services. Utilizing virtual platforms, you will engage with the child and their support team to ensure a holistic evaluation. Leveraging the Child and Adolescent Needs and Strengths (CANS) Assessment tool, you will produce detailed and thorough written evaluations. All assessments must be completed within a 30-day timeframe, ensuring timely and effective support for each child's needs.

Please Note: We have recently filled all openings in Mississippi; however, we currently have three remaining positions available for candidates residing in West Virginia. As this is a hybrid role in West Virginia, some travel may be required.

Job Responsibilities:

  • Conduct comprehensive member assessments within specified timelines, ensuring meticulous documentation throughout the process.
  • Accurately and promptly input assessment data into the web system.
  • Participate in inter-rater activities to ensure consistency in the administration of assessment instruments and adherence to protocols.
  • Provide training to provider agencies and other stakeholders as needed or assigned.
  • Manage special projects as assigned, demonstrating adaptability and a proactive approach.
  • This role may require travel approximately 10% to 20% of the time.

The list of accountabilities is not intended to be all-inclusive and may be expanded to include other duties that management may deem necessary.

Required Qualifications/Experience:

  • Minimum of 2 years' experience in assessment of children who have exposure to trauma, mental health issues and/or physical difficulties.
  • Understanding of Child Welfare, Behavioral Health, and Medicaid policies and procedures.
  • Bachelor's Degree in a related healthcare field.
  • Proficiency in person-centered planning fundamentals and knowledge of child welfare policies and procedures.
  • Proficiency in Microsoft Office and advanced computer skills, including the ability to navigate multiple databases, software applications and medical databases and perform data entry and documentation.
  • Flexible and eager to learn new skills and tasks.
  • Excellent communication skills (written, verbal, and listening), effective interpersonal and a professional demeanor.
  • Strong organizational skills and the ability to manage multiple tasks simultaneously.

Compensation

The compensation for this role is $57,500 base salary

Based on our compensation program, an applicant's position placement in the pay range will depend on various considerations, such as years of applicable experience and skill level.
