Job Description
POSITION SUMMARY: The Welcome House Psychiatric Rehabilitation Worker provides psychiatric rehabilitation with a recovery-oriented rehabilitation program model for individuals with serious and persistent mental illness and co-occurring disorders. DUTIES AND RESPONSIBILITIES:
- Works in conjunction with other psychiatric rehabilitation staff in on-site Rehabilitation-Based Services (RBS) to help facilitate client progress toward recovery-based rehabilitation goals by facilitating psycho-educational and skill building activities, both on site and in the community, that address client life domains, which include but need not be limited to:
- physical and psychological wellness
- activities of daily living
- instrumental activities of daily living
- interpersonal skills and relationship-building
- prevocational readiness and community connectedness, and
- cultural and spiritual development
- Promotes community integration by assisting individuals to learn about and utilize community resources, accompanying participants individually or in groups to facilitate community participation.
- Provides linkages to resources in the community, including but not limited to volunteer work, employment opportunities and community resources, to assist individuals to achieve and maintain their chosen individual recovery goals
- Maintains programmatic philosophies and ideals (e.g. service delivery, required documentation, code of ethics, processes and practices) as stated in Psychiatric Rehabilitation principles/regulations for Pennsylvania, which include but need not be limited to promoting self-determination and empowerment, conveying hope and respect that all individuals have the capacity for learning and growing and providing person-centered treatment to ensure that consumers improve quality of all aspects of their lives (social, wellness, occupational, educational, residential, intellectual, spiritual, and financial) and build upon their strengths and capabilities
- Develops and maintains clinically informed, culturally sensitive, and therapeutically valid relationships with consumers by working one-on-one and engaging the consumers in group activities to aid consumers with skill development and increasing their ability to transfer learned skills to their home and communities so that they are able to make strides towards their individual recovery goals and overcomes barriers
- Completes clinical documentation related to consumer-centered goals as specified in the Individual Recovery Service Plan by completing a readiness assessment, developing goals, and planning appropriate services with each participant. The case record must include progress notes for each encounter (e.g., in-person, telehealth) in the community and each day on site
- Coordinates daily outreach activities to include contacting absent members, corresponding in writing when unable to contact by telephone for documentation and continuation of consumer engagement
- Works collaboratively with other service providers, support systems, and social service programs (e.g., case manager, family members) utilized by the members for the facilitation of Psychiatric Rehabilitation activities such as group participation, developing and working towards individual recovery goals and developing personal support networks to aid the consumer with overcoming barriers so they can apply skill transference to their home and community
- Actively participates in the individual and team supervision process through activities including but not limited to coming prepared with an agenda, providing a case load summary, and reviewing training opportunities/ initiatives; immediately notifies their supervisor of any pertinent information (e.g., decompensation of consumer, major changes to a consumer's life, loss of insurance) to ensure continuity and quality consumer care
- Stays informed of all program updates, news, and changes using email and verbal communication with Director, Program Supervisor, and other staff
- Assists in evaluating program needs through meetings and feedback to the supervisor and making recommendations for program development and expansion, including input on facilities' needs and purchases for improvement of the program and facility to ensure programming needs are being addressed
- Performs cardiopulmonary resuscitation (CPR), first aid, and crisis intervention using agency-trained protocols
- Performs other duties as assigned
IMMEDIATE SUPERVISOR: Program Supervisor DIRECT REPORTS: None CONTACTS: Clients, client families, and other supports, Elwyn staff, various community stakeholders (such as employers, county representatives, other agencies, etc.) EDUCATION/EXPERIENCE/SKILLS REQUIREMENTS:
- As stipulated in 55 Pa. Code *5230.51(c) of the Psychiatric Rehabilitation Services regulations, must have:
- Bachelor's degree, or
- Associate's degree and one (1) year work experience in mental health direct service, or
- Certified Peer Specialist (CPS) certificate and one (1) additional year paid or volunteer work experience in mental health direct service, or
- High school diploma or equivalency and two (2) years' work experience in human services which must include one (1) year of mental health direct service
- Demonstrated ability to work effectively as part of a team
- Ability to work in a fast-paced environment and manage and prioritize multiple, often competing, priorities
- Demonstrated strong attention to detail
- Demonstrated strong time management and organizational skills
- Demonstrated excellent judgment with the ability to independently solve problems and make decisions with little or no need for direct supervision
- Demonstrated ability to anticipate future needed changes or identify problem areas and take effective actions
- Must be able to perform CPR and crisis intervention using agency-trained protocols
- Must possess excellent customer interaction, collaboration, presentation, and written and verbal communication skills
- Demonstrated basic experience with Microsoft Office applications, including Word, Excel, and Outlook
- Knowledge of Electronic Health Record (EHR) / Electronic Medical Record (EMR) preferred
- Must have current, valid driver's license in state of residence, three (3) years driving experience in the United States, and acceptable driving record
