Job Description

$2000 Sign on Bonus for New Hires

Pay Rate: $20/HR

Shift Details

8:00am-4:00pm Monday-Friday

POSITION SUMMARY: The Program Specialist provides case management for a group of individuals served in Senior Adult Day Programs. This position liaises with families, agencies, and Elwyn personnel; participates as a team member to advocate for the individual; implements Care Plans as written; communicates changes in needs for individuals; supervises the implementation of Care Plans; and secures all needed services.

DUTIES AND RESPONSIBILITIES:



  • Complete required documentation including but not limited to annual Care Plans and six (6) month reviews, monthly progress notes, quarterly fire safety training for individuals, incident reports, and individual records for the program to ensure regulatory compliance
  • Ensure compliance with Pennsylvania Department of Aging, PA Chapter 11 Regulations as follows by:
  • Collecting all pre-admission paperwork, such as physical (completed within 12 months), admission application (completed within 3 months), tetanus diphtheria within 10 years, tuberculosis screening within 2 years, and other pertinent information
  • Scheduling and completing an interview and tour of the program with individual
  • Admitting the individual and completing admission paperwork, such as authorizations, on first day of admission
  • Developing and writing the initial Care Plan within 30 days of admission, based on medical diagnosis and support needs, to include, but not limited to, listing concerns (medical diagnosis), goals, person responsible for implementing and documenting, method to achieve the goal, and time frame
  • Scheduling and completing meeting with plan team members within 30 days of admission to review the Care Plan and make any changes if necessary, and completing team sign-in sheet to record attendees
  • Developing and writing annual Care Plans and six (6) month reviews, based on medical diagnosis to include but not limited to listing concerns (medical diagnosis), goals, person responsible for implementing and documenting, method to achieve the goal, and time frame
  • Attending the Care Plan/Individual's Program Plan (IPP)/Individual Support Plan (ISP) meetings to advocate for the individual, to be part of the team that makes decisions regarding how to care for the individual and to use information discussed during meeting to identify strengths and needs to develop and write the annual Care Plans
  • Ensuring the implementation of the Care Plans by staff and all responsible parties, including themselves, as written, through activities including but not limited to elevating legs, keeping glasses clean, monitoring while eating, changing adult protective undergarments every several hours
  • Supervising, monitoring, and evaluating services provided to the individual through tasks including but not limited to collecting monthly progress notes from direct care staff and reviewing for accuracy and progress on goals, observing direct care support provided by staff to the individual and giving instruction or feedback to staff as needed, as well as training staff in the IPPs and Care Plans contents, which includes the support and supervision needs of the individual
  • Reviewing, signing, and dating the monthly documentation of an individual's participation and progress toward goals outlined in the Care Plans

    • Reporting a change related to the individual's needs to the Qualified Intellectual Disabilities Professional (QIDP), as applicable, and plan team members for the purpose of having the individual receive medical treatment and support as needed


  • Reviewing the Care Plans with the individual to ensure understanding
  • Documenting the review of the Care Plans with the individual by signing and dating the Care Plans, and entering the date of the next reviews
  • Ensuring annual individual physical examination form and quarterly nursing assessments are received from the Nurse Case Manager and placed in file
  • Coordinating the services provided to an individual through tasks including but not limited to contacting the QIDP, Physical Therapy, Nurse Manager and/or clinician to provide training or evaluation or required information to provide services to the individual
  • Coordinating the training of direct service workers in the content of health and safety needs relevant to each individual with tasks including but not limited to arranging for professional staff to train the direct service workers regarding the needs of the individual, to ensure understanding of the plan, and that goals are being implemented correctly
  • Communicate with team members, agencies, and community members on behalf of the individual through phone calls, email, and meetings to meet their needs and regulatory requirements, including communicating with internal team members such as QIDP, Nurse Case Manager, and residential building management about health concerns with individual
  • Initiate contact with the QIDP, when needed, to schedule an internal or Special Team Meeting or when individual's needs have changed, to ensure that the changes are updated in the individual's record throughout the full file
  • Participate in the development of IPPs and ISPs including annual updates and revisions by providing content input, such as the strengths and needs of the individual, concerns, and observations, for accuracy and reporting content discrepancy to the QIDP, and plan team members as applicable
  • Ensure the ongoing accuracy of the Care Plans by making necessary changes to the Plans as needs of the individual change, and the implementation of the Care Plans as written by communicating and training staff in any changes in needs of the individual
  • Provide informal counseling to individuals, as needed, concerning changes in life, including but not limited to changes in their health and well-being or life-event changes to provide support and adjustment to the changes
  • Provide Fire Safety training to individuals on a quarterly basis, including an assessment of ability to participate in fire safety training with individual, and complete quarterly Fire Safety form for each individual
  • Complete incident reports for physical or psychological injury to individual, or anything that would lead to an injury
  • Provide back-up for other program staff as needed, such as provide direct care to individuals or be the primary leader and decision-maker within the program in absence of other leadership
  • Participate in emergency response situations, including but not limited to calling 911 in a medical emergency and perform CPR or first aid, in accordance with established program protocol, and may need to accompany individual to hospital if needed
  • Perform cardiopulmonary resuscitation (CPR), first aid, and crisis intervention techniques using agency-trained protocols
  • Perform other duties as assigned

CONTACTS: Elwyn Staff, individuals supported, families/advocates, funding sources, residential agencies, Behavior Specialists, Support Coordinators, members of oversight organizations such as Adult Protective Services

EDUCATION/EXPERIENCE/SKILLS REQUIREMENTS:



  • High school diploma or equivalent; bachelor's degree in Special Education, Psychology, Public Health, Rehabilitation, Social Work, Speech Pathology, Audiology, Occupational Therapy, Therapeutic Recreation or other human services field preferred
  • Two (2) years of experience working with individuals with intellectual and/or developmental disabilities and/or medical support needs
  • Case management experience preferred
  • Professional writing skills; understanding of medical terminology/language and experience working with people with medical needs; ability to follow physician orders
  • Knowledge of the program regulations (Aging, Chapter 11) preferred
  • Demonstrated ability to work effectively as part of a team
  • Ability to work in a fast-paced environment and manage and prioritize multiple, often competing, priorities
  • Demonstrated strong attention to detail
  • Demonstrated strong time management and organizational skills
  • Demonstrated excellent judgment with the ability to independently solve problems and make decisions with little or no need for direct supervision
  • Demonstrated ability to anticipate future needed changes or identify problem areas and take effective actions
  • Ability to perform CPR, first aid, and crisis intervention techniques in accordance with agency trained protocol
  • Must possess excellent customer interaction, collaboration, presentation, and written and verbal communication skills
  • Demonstrated intermediate experience with Microsoft Office applications, including Word, Excel, and Outlook
  • Must have current, valid driver's license in state of residence, three (3) years driving experience in the United States, and acceptable driving record
