We are looking to add a full-time Clinical Care Coordination Supervisor to our talented team of Social Work professionals.

Summary:
The Clinical Care Coordination Supervisor (C3 Supervisor) oversees daily activities and provides administrative and operational support to Clinical Care Coordinators (C3s). The position will monitor patient admission, placement, flow, CCM staffing, service department testing and procedures, and conduct analysis of clinical information, performance metrics, and identify opportunities for improvement. In partnership with physicians, ensures accurate patient status and appropriate patient placement for effective, efficient patient focused outcomes. Will work collaboratively with all members of the healthcare team to improve patient throughput and length of stay (LOS) performance of the organization.

Job Responsibilities:



  • Provides daily leadership, direction, resources, training and education, evaluation and support to C3s.
  • Assists Care Coordination leadership in identifying, monitoring, and evaluating department operations to ensure optimal efficiency and effectiveness.
  • Audits patient plan of care assessments and care plan development for completeness and timeliness according to state requirements.
  • Serves as escalation point for Clinical Care Coordinators (C3s) to address barriers that may impede system-wide flow.
  • Provides leadership for continuous performance improvement and facilitates clinical decision making through ongoing monitoring of patient outcome indicators.
  • Rounds on patient care units to ensure patient progression and flow.
  • Builds and maintains collaborative relationships between unit and service area team members and the Care Coordination model.


Education:



  • Bachelor's Degree in a health related field required


Experience:



  • Three (3) or more years' clinical experience required
  • Supervisory and managerial experience preferred
  • Nurse Case Management experience preferred


Skills and Certifications:



  • Registered Nurse in the State of Georgia required
  • Basic Life Support (BLS Certification) for Critical Care and ECC units Advance Cardiac Life Support (ACLS) is required.
  • Certified Case Manager (CCM) or American Case Manager (ACM) Certification preferred
