The Division of Hospital and Palliative Medicine at the Keck School of Medicine of USC seeks an exceptional Advanced Practice Provider (APP) to join our division and Enhanced Skilled Nursing Facility (SNF) Program.

This is a unique full-time position that will have an emphasis on caring for discharged patients from Keck Medical Center of USC who are recovering at multiple USC partnered skilled nursing facilities. The Enhanced SNF Program aims to medically manage our patients during their stay at skilled nursing facilities while focusing on quality, enhancing continuity, and improving transitions of care throughout the healthcare continuum.

Applicants should demonstrate a dedication to caring for a diverse patient population, excellent communication skills, and an enthusiasm for working in a collegial interdisciplinary team-based environment.

Clinical responsibilities for this role may include but are not limited to obtaining patient histories, complete physical examinations, documentation of findings, ordering and interpreting of diagnostic and laboratory tests, consulting with other providers when appropriate, and helping to facilitate a safe and efficient operational flow of the patient through the continuum of care. The APP will function as a leader within our professional team by communicating, planning, and implementing care directly with other healthcare professionals including but not limited to physicians, pharmacists, nurses, and other advanced practice providers.

This position will have an anticipated start date of late fall/early winter 2024.

This position offers a base salary of $150,000-$200,000, depending on the applicant's professional experience. When extending an offer of employment, the University of Southern California considers factors such as (but not limited to) the scope and responsibilities of the position, the candidate's work experience, proposed academic rank at the time of hire, education/training, key skills, internal peer equity, federal, state, and local laws, contractual stipulations, grant funding, as well as external market and organizational considerations.
