Purpose of Job

This position is responsible for providing nursing care under the supervision of the Registered Nurse in accordance with the Standards of Nursing Care and Practice as defined by Ridgeview Medical Center and the Minnesota Nurse Practice Act.

Job Functions



  1. Collects and monitors patient health data and provides patient care under the direction of the Registered Nurse.


  2. Conducts a focused assessment of the health status of an individual patient through the collection and comparison of date to normal findings and the individual patient's current health status, and reports changes and responses to interventions in an ongoing manner to a registered nurse or the appropriate licensed health care provider for delegated or assigned tasks or activities.


  3. Reinforces previous education provided to patients, family members and significant others on health maintenance and disease prevention for the patient under the direction of the Registered Nurse.


  4. Documents and maintains accurate medical records and reports.


  5. Serves as a patient advocate in seeking appropriate community resources to meet patient needs.


  6. Demonstrates competencies in the specialty area of Home Health Home Care.


  7. Performs other duties as assigned.


MinimumEducation/Work Experience


  • Graduate of an accredited school of practical nursing

Knowledge/Skills/Abilities



  • Demonstrate critical thinking and decision making


  • Ability to meet and maintain the necessary background checks as aligned with position functions


  • Ability to communicate in the English language for effective written and verbal correspondence in order to complete job functions as mentioned above


  • Ability to work autonomously and as a team


  • Capable of making independent decisions with the use of good critical thinking and problem solving skills within the scope of practice


  • Ability to work and multi-task in a high stress and fast paced environment


  • Ability to function both independently and as part of a team


  • Ability to respond appropriately to unpredictable situations and diverse patient populations


  • Ability to communicate in the English language for effective written and verbal correspondence in order to complete job functions as mentioned above


License/Certifications


  • Current Practical Nurse licensure in Minnesota or if licensed in another state, a permit from the Minnesota Board of Nursing while the license application is in process
  • Current Healthcare Provider CPR certification
  • Valid MN driver's license and meets Ridgeview's insurability requirements

Preferred Qualifications


  • Previous nursing experience in a hospital or similar healthcare setting
