Program Overview Community Solutions, Inc. is seeking a dedicated and passionate Discharge Planner/Correctional Case Manager to help serve our Santa Maria and Santa Barbara in Custody program. In this role you will facilitate positive change in client behavior through the use of individually tailored service plans based on assessments made through the use of motivational enhancement strategies. You will work with the CSI team to communicate direction and information and assist the State Director in maintaining a positive relationship with funder and assuring contract fidelity. This role will include working and traveling to both the Santa Maria and Santa Barbara locations spending one week at a time in each location, mileage and travel time is paid. Hourly Rate: $30.00 Position Overview
- Share strength and needs between the funder staff and CSI staff. Partner with the Area/State Director to determine action steps for continuous improvement.
- Function as the team leader by providing support, direction, and clarification of contract requirements to all CSI Correctional Counselors.
- Train new CSI Correctional Counselors as needed.
- Ensure quality of program delivery.
- Review all client required documentation regarding content, timeliness and quality and report findings to supervisor.
- Conducts risk and needs assessment interview in order to develop an individual service plan that will appropriately meet the short and longer-term needs of the client.
- Coordinates and supports the efficiency of access to mental health support and services via internal jail resources and community-based services.
- Acting as a conduit of risk and need information, treatment recommendations, and housing and to all units needing information about risk and needs to assist in the reduction of client jail time as guided by best practices in the Criminal Justice system.
- Coordinates client's participation in evidence-based, curriculum driven groups to educate and assist clients in recognizing and understanding the nature of the problem to assess the need for treatment and services, and to evaluate progress.
- Develops and implements individualized treatment and service plans based on results from validated risk and need assessment tools.
- Works closely with Discharge Planning team to make referrals for County health services as needed, confers with probation, parole, and other law enforcement agencies to determine clients legal status and/or develop appropriate discharge plans.
- Consults with representatives from community treatment programs and recovery facilities to determine the appropriate placement of clients and to ensure continuity of care after release from custody.
- Maintains robust documentation in CSI and Funder Case Management systems, writing reports and summaries, making recommendations, and documenting client progress; completing forms and maintains client records and charts according to established procedures, guidelines, and legal requirements.
Qualifications:
- Bachelor's Degree plus one (1) year experience in social services field. Education maybe substituted with relevant experience on a one year for one year basis.
- One year experience as a team lead or other supervisory experience preferred.
- One year experience in teaching, training and/or group facilitation.
- Excellent oral and written communication skills
- Valid driver's license and reliable transportation to go to Santa Maria and Santa Barbara locations.
