Job Description: Matt's Place Day Treatment is an 8-10 week program, that runs M-F 8:30-3pm, that consists of two hours of schooling per day, groups, individual therapy, medication management, and recreational therapy. We are a center for the benefit of individuals living with social and behavioral struggles and autism spectrum disorders.The Clinical Mental Health Professional utilizes clinical expertise to perform psychosocial assessments, develop and implement plans of care appropriate care team, assess crisis situations to appropriately provide needed resources and or referrals for the patient, and to provide family and individual treatment as indicated. This position works collaboratively with patients, patient's significant other, families, healthcare providers, insurers, community resources, and all other involved parties. This position provides care appropriate to the established plan of care in the assigned patient population. We are a center for the benefit of individuals living with social and behavioral struggles and autism spectrum disorders. The Clinical Mental Health Professional will assess the patient's medical history, mental health history, and current presentation to understand the patient's needs from treatment, and then develop a plan of care either individually or with input from the patient, patient family and significant others, physicians, and other members of the healthcare team. Our team is highly collaborative, team-focused, and supportive. We are committed to learning and sharing new knowledge in our field, and integrating such knowledge into our program. We place high value on strong communication and utilizing expertise of all members of the team in maximizing treatment outcomes. Cases are staffed weekly by an interdisciplinary team and all members of the team work closely with each other throughout the week to ensure patients' needs are met and the treatment plan is appropriate.
